Health Center Controlled Network (HCCN) Program is a grant from the Health Resources and Services Administration (HRSA) that funds networks of federally qualified health centers to leverage health IT and data to improve primary healthcare delivery.

HRSA will award approximately $48 million to fund 49 HCCNs, which must include at least 10 participating health centers that are majority controlled and at least majority owned by Health Center Program award recipients. Program activities focus on data management and analytics, health system interoperability, digital health tools, and UDS+ data submission.

Eligible applicants include currently funded or newly formed HCCNs, public or private nonprofit entities, institutions of higher education, and Native American tribal groups.
