Health Systems Access to Care Fund » Oregon Community Foundation Health Systems Access to Care Fund Kaiser Permanente NW, Legacy Health, PeaceHealth and Providence Health & Services collaboratively created the Health Systems Access to Care Fund for community-supported clinics in the combined service areas of Oregon and SW Washington.
The goal of the fund is to strengthen the capacity and infrastructure for clinics that are expanding and/or adapting their current models to respond to the changing needs of patient populations due to ongoing healthcare reform, Medicaid transformation, and the increasing pressure on the safety net for access to care. Invited applicants are Community Supported Clinics in Oregon and SW Washington, within the service areas of the Health System partners. A community-supported clinic is defined as: 1. A non-profit, locally based healthcare provider that serves people who may experience barriers to getting care elsewhere (FQHCs, community health clinics, or large health systems) with a strong emphasis on providing care for the uninsured.
Their mission is articulated, their practice model is aligned with their mission, includes place-based strategies and a focus on serving an at-risk population. 2. The clinic and its model of care is a.
not adequately supported by “fee for service” insurance billing b. not at a scale to participate in value-based-pay funding arrangements 3. Funding model includes a.
broad community support – more focus on local funding (grants and major donors) to support the aspects of their mission-driven practice/model of care b. attracting and retaining volunteer clinical and non-clinical staff who share a commitment to the mission and model of care. Eligible organizations must also: provide primary medical care and/or primary dental care access (not a specialty care provider).
have referral arrangements in place for broader care and service needs of people they serve meet or exceed community standards of practice in primary care for the scope of services provided, with a culture of continuous improvement. Federally Qualified Health Centers (FQHCs) are not eligible to apply. No major capital requests will be considered.
