The child must be 21 years of age or younger (an individual aged 22–25 may be considered if they are receiving pediatric treatment).

The child must be under the care of a physician. The request must be clinically relevant to a specific healthcare need of the child. There must be no existing insurance coverage for the requested expenses.

Only one request per 12 months, per individual, is accepted, for a maximum of three times in an individual’s lifetime. Pediatric grants for clinical and equipment requests average US$2,500 and do not exceed US$17,000. Grants for travel and lodging requests do not exceed US$1,000.

Financial eligibility based on qualifying income: To calculate your qualifying income, reference the first page of your most recent federal income tax return, take your adjusted gross income (AGI), and subtract out-of-pocket medical expenses for the child in the last 12 months. The sum will equal your qualifying income. See the table on the right.

We always consider the adjusted gross income for both parents/guardians, if applicable. If parents/guardians are separated, we will request the adjusted gross income of the parent claiming the child as a dependent.

For applicants outside the United States, please provide your household’s most recent official annual income documentation (such as tax forms, payslips, or a letter from your employer), and subtract your child’s out-of-pocket medical expenses for the last 12 months.

Qualifying income cannot exceed (in US$) Lodging (only if related to a child’s care) Transportation, gas, charging, parking, etc. related to a child’s care Travel within the United States, only for a child who is a resident of the United States Vehicle purchases or repairs Travel for a child who is not a resident of the United States Clinical: Procedures, medicine, evaluations, therapies, etc. Note on therapy requests: Max of 26 sessions of weekly therapy (approximately six months) will be considered Max of three weeks of intensive therapy will be considered Equipment: Hearing aids, wheelchairs, assistive technology, prostheses, etc. Requests for debt reduction or past medical bills, copays, or deductibles Requests for research funding Permanent home modifications Education-related therapies or expenses Alternative or experimental treatments and therapies whose effectiveness or safety remains a topic of debate and controversy among medical professionals If you are seeking care with an out-of-network provider, please note the following: Out-of-network requests must have a strong medical justification; factors considered may include continuity of care, distance to care, and wait times for in-network care.

A parent/guardian must provide a letter of justification for choosing an out-of-network provider. If you have specific questions, email [email protected] . Physician documentation must be submitted in letter form, on letterhead, outlining the child’s diagnosis and explaining the medical necessity for the specific request for funding.

In addition to a letter from a doctor, you may also include an evaluation from a specialist (e.g., an audiologist, therapist, etc.). If your child has not received an evaluation from a specialist, upload a duplicate copy of your physician letter in lieu of an evaluation, as the grant application requires a document to be uploaded here.

If the funding request is for travel/lodging, your provider/social worker should outline the expected dates/duration of treatment in letter form, on letterhead. We are unable to accept the following documentation in place of a letter: patient portal chart notes, prescriptions, Individualized Education Program (IEP). Upload the first page of your most recent federal income tax return or W-2.

If you have not filed taxes, please submit three months of bank statements and/or a letter from your employer verifying your annual income; if uploading bank statements, please ensure the monthly summary page showing total deposits is included.

For applicants outside the United States, please provide your household’s most recent official annual income documentation, such as tax forms, payslips, or a letter from your employer verifying your annual income. All clinical and equipment requests must be accompanied by a quote from the provider/equipment vendor. All quotes must clearly state the price and the name and mailing address of the provider/equipment vendor.

Ensure the quote includes any applicable discounts; applications without discounts may be delayed or declined. For clinical treatments involving multiple sessions (e.g., therapies), the quote must specify the total number of sessions. Note on therapy requests: A max of 26 sessions of weekly therapy (approximately six months) will be considered.

A max of three weeks of intensive therapy will be considered. Insurance exclusion or denial verification Insurance exclusion or denial of treatment/equipment must be submitted in letter form, on letterhead. There must be no insurance coverage for the requested expenses.

Copays or deductibles are not eligible for funding. Oracle Health Foundation will reach out if additional information is needed. Applicants will be notified of the funding decision within approximately one month of the application being submitted.

If a grant is approved, payment will be sent to the provider/equipment vendor (not to the family) within approximately one week of approval. Grant funds must be used within 12 months of the date approved. Oracle Health Foundation reserves the right to distribute funding at its sole discretion.
