Healthcare Technology Program (Central California Alliance for Health) is sponsored by Central California Alliance for Health. This program supports organizations serving Medi-Cal members in specific California counties (Mariposa, Merced, Monterey, San Benito, Santa Cruz) to improve access to high-quality healthcare through technology.

Grants fund the purchase and implementation of technology and infrastructure, such as platforms for patient engagement and care coordination.

Healthcare Technology Program - Central California Alliance for Health Medi-Cal Capacity Grant Program Healthcare Technology Program The Alliance makes investments to improve coordination across the health care system and address infrastructure and capacity gaps to ensure that Medi-Cal members are able to access high-quality care when, where and how they need it.

The Healthcare Technology Program aims to improve Medi-Cal member access to high quality health care for the Medi-Cal population in the Alliance service areas. Applications are currently being accepted from Mariposa, Merced, Monterey, San Benito and Santa Cruz counties. You may only have one grant-funded project at one time under this program.

Expand capacity for telehealth and e-Consult for Medi-Cal members. Healthcare Technology Program grants support the purchase and implementation of specific types of technology and infrastructure that expand capacity for organizations that serve the Medi-Cal population. Types of funded projects are listed below.

Platforms for patient engagement and care coordination Implementation of platforms that: Support member convenience in accessing care and managing health conditions. Support health care providers’ operational efficiencies to expand capacity for health care service delivery. This includes technology such as: Patient monitoring devices.

Patient intake and assessments software. Technology that supports cultural and linguistic competent health care. Software that supports information sharing for patient care.

Implementation or expansion of telehealth or e-Consult services. I mplementation of medical equipment that is necessary for Medi-Cal service delivery and directly relates t o specific quality measures targeted for improvement. Examples include lead analyzer s , HbA1 analyzers, diagnostic equipment and vaccine refrigerators .

The Alliance will not fund the following: Equipment already purchased or in place. Equipment reimbursed through the Alliance’s authorization process. Duplication of infrastructure and services provided by the Alliance’s Telehealth Services .

Strengthen your application Applicants who demonstrate one or more of the following will strengthen their grant application: How proposed technology/infrastructure is a solution to barriers to care and/or supports the delivery of coordinated and integrated care. Use of evidence-based practices or recognized effective practices. Leveraging other funding sources and initiatives.

Metrics to evaluate effectiveness of technology after implementation. Applicants must meet the minimum eligibility requirements for the Medi-Cal Capacity Grant Program (see Eligibility Guidelines ). Applicants must also be one of the following: Contracted Alliance provider.

Eligible applicants must serve a significant volume of Medi-Cal members in the Alliance service area. The Alliance will consider funding projects up to $50,000 per grant. Project timelines should be no more than 12 months.

Project timelines are defined as the date the project scope of work begins (after grant award date) to the date of project completion. An applicant is limited to Healthcare Technology funding for one project at a time which may be implemented at more than one site. A project budget may contain costs for more than one site.

Funding will be considered for projects already in progress; however, activities completed or costs incurred prior to approval of the grant request by the Alliance will not be considered. The Healthcare Technology Program awards one-time grants. Funding for ongoing subscription, license or maintenance fees or other operational costs will not be considered.

Applicants must demonstrate secured financial commitment to support the full cost of the project from other sources beyond the grant award. Useful life of technology The useful life of technology should be at least three years. Applicants must plan to use the technology for the benefit of Medi-Cal members for a minimum of three years after completion of implementation.

Examples of eligible expenses include: Consultants and vendor costs for project start-up and/or implementation of technology. One-time purchase of equipment and/or technology. Training of health care provider staff and/or patients in use of technology.

Applications require submission of financial statements, including: Audited financial statements for the last two fiscal years. An organizational profit and loss statement and balance sheet for the last 12 months.

If audited financial statements are not available, applicant should provide a rationale for not performing an annual audit and upload their company or organization’s most recent tax return or Form 990.
