HealthWell Foundation Disease Management Grants is a patient assistance program from the HealthWell Foundation that funds out-of-pocket medical costs for individuals living with serious, chronic, or life-altering diseases. The program provides financial assistance to help patients afford medications, insurance premiums, co-pays, and other treatment-related expenses.
Eligible applicants must have health insurance coverage and household income up to 500% of the Federal Poverty Level. Grants of up to $15,000 are available to qualifying patients receiving treatment in the United States. If you have not received a response from our team or your submitted documents show as pending in your portal, please know these are in queue to be reviewed in the order they are received.

Our Re-enrollment Funds are open only to patients who have had and used a grant with HealthWell in the past. While we continue to manage the existing grants in our Closed Funds , they are temporarily closed to new patients due to lack of sufficient funding. Please continue to visit our Disease Funds page often, as replenished funds reopen as quickly as possible.
If you currently have a grant with HealthWell, your grant will remain active for the entire 12 month grant cycle or until you have exhausted your allocated grant amount, whichever comes first.
