Healthy Communities Initiative – Pallottine Foundation of Huntington Healthy Communities Initiative the healthy communities initiative closed feb. 24, 2026. The Healthy Communities Initiative pr ovi de s funding and support to nonprofit organizations administering health-related programming to address the behavioral, emotional, physical, social, and spiritual needs of the community.
The initiat ive also supports projects that improve the nonprofit’s internal capacity to work effectively and develop strong finances, leadership, and infrastructure. Healthy Communities is open annually and has a one-year grant period with awards up to $40,000. Applications may include funding requests for multiple projects and contain both capacity building and health and wellness components.
The Healthy Communities Initiative consists of two application pathways. Only one application per organization is permitted. Applicants seeking funding totaling $7,501 to $40,000 should complete the standard application form , which requests traditional organizational financial statements.
Applicants are encouraged to use this form for more detailed programming and capacity-building projects. For smaller requests totaling $7,500 or less, applicants should complete the simplified application form , which provides a condensed question list and more flexible financial statement requirements.
First-time applicants, emerging nonprofits or organizations with nontraditional financial statements are encouraged to select this pathway. It also may be a good fit for organizations seeking support for basic needs, capacity building, or small-scale programming. June 1, 2026 - May 31, 2027 Review the eligibility guidelines to ensure your organization meets requirements. A note regarding nonprofit status: The Foundation recognizes that impactful work often begins before formal nonprofit status is established.
Organizations that do not yet have a nonprofit designation may apply for funding from the Foundation utilizing an eligible fiscal sponsor, provided both the fiscal sponsor and the sponsored organization and its project align with the Foundation’s mission and eligibility requirements. A fiscal sponsor is an IRC 501(c)(3) organization that agrees to receive and manage grant funds on behalf of another organization.
Any organization that intends to utilize a fiscal sponsor should contact a member of the Foundation’s program team before beginning an application . Review the following pages to ensure your proposed project aligns with a focus area: Mental and behavioral health Tobacco use prevention and cessation Review the standard application form and simplified application form to learn about the types of information requested.
Organizations may submit only one form per grant cycle. Applicants will be asked to provide financial information and detailed organization and project overviews. Applicants may submit a project budget in their preferred format or download the Foundation’s project budget template and sample project budget .
Complete and submit your application by 8 p. m. on the deadline date.
Late applications will not be accepted. After the application period closes, Foundation staff members will review applications and may request additional information or a site visit. Following the review process, the Foundation’s Board of Trustees will make funding decisions and applicants will be notified by email.

We encourage potential applicants to contact us before and during the application process to discuss project ideas, share information, and ask questions.
