Home Health Aide (HHA) Track: Grant Application Home Health Aide (HHA) Track: Grant Application Use this application to express interest in being part of the Pennsylvania Homecare Associations' (PHA) Home Health Aide (HHA) Track of the Training and Mentorship Program funded by the Department of Labor and Industry Job Quality Grant.
Eligibility Requirements: Desire to increase skills Currently employed by home care, home health or hospice agency OR employment pending training Applicants must work 40 hours in the industry to receive grant funding Applicants must work in Pennsylvania Step 1 : Complete this application to apply to be considered for the grant program Step 2: Complete the MyLearning Center Training Videos within 30-days of completing this application Step 3: Complete 16-hour classroom training Step 4: Accept a job offer with a home care, home health or hospice agency and work your first forty (40) hours.

Please note - the above steps must be completed within 30 days of application to be awarded the training stipend. By successfully completing the above items, you will be awarded $500.

I understand that if I fail to complete the videos in this time frame, I will need to reapply. * I agree to complete 16-hours of face to face training * I agree to apply to at minimum three (3) jobs using the PHA job board and provide documentation that I was hired and worked forty (40) hours at home care, home health, or hospice agency * I agree to complete a satisfaction survey at the conclusion of the training program.
