Services listed below are subject to change based on special grants and funding availability Caregiver Support Coordination Caregiver Respite In Home Emergency Response Button Health & Wellness Initiatives Older Relatives Caregiver A service to assess the needs of an older person and effectively plan, arrange, coordinate and follow-up on services which most appropriately meet the identified needs as mutually defined by the older person, the access and assistance staff, and where appropriate, a family member(s) or other caregiver(s).
Caregiver Support Coordination Our purpose is to assist seniors, their family members, and/or persons with a disability in accessing appropriate and needed services. We do have a screening process and assist the highest needs. Assists family caregivers with information, training, services, and support programs.
Services include Respite care through a paid provider or someone the family may know. This provides caregivers a chance to run errands and have relief from burnout. If you are 18 or older and are caring for anyone over 60, or are a relative caregiver 55 or older raising a relative child under 18, you may qualify for support.
Assist caregivers with services or goods that support the basic needs of the individual on behalf of their caregiver. For those 55+ raising a child, such as a grandparent, or family member raising a grandchild, we can also assist with income assistance for school supplies, school clothes, and summer camps.
Counseling or representation services provided by a benefits counselor, where permitted by law, to Medicare beneficiaries, family members, caregivers or othersworking on behalf of an eligible person.
An eligible person is one of the following: A new to Medicare enrollee A dually eligible Medicare beneficiary A beneficiary who is disabled as determined by Social Security Administration criteria A person assisting a Medicare beneficiary and the person receives assistance related to a Medicare or State Health Insurance Assistance Program (SHIP) topic or both.
Eligibiltiy – Must be eligible for (or on) Medicare or Medicaid, or an advocate, (POA) required, for someone who is on or eligible for Medicare or Medicaid.
Caregiver Respite In Home Temporary services for an eligible dependent care recipient for the relief of a caregiver provided in the eligible caregiver’s home or the home of the care recipient on a short term, temporary basis while the primary caregiver is unavailable or needs relief. In addition to supervision, services may include meal preparation, housekeeping, assistance with personal care, and social and recreational activities.
A service provided through the consumer directed service option which allows the caregiver to choose an individual provider for the following types of respite care: Services are provided on an intermittent or temporary basis while the primary caregiver is unavailable or needs temporary relief.
A service to perform heavy household tasks which an older person is not able to perform on their own, such as heavy cleaning (e.g., scrubbing floors, washing walls, or washing windows inside and outside), moving heavy furniture or maintenance such as yard or sidewalk maintenance.
Emergency Response Button Services for an older person who is homebound or frail to establish an automatic monitoring system which links to emergency medical services when the person’s life or safety is in jeopardy.
Emergency Response System (ERS) services include the installation of the individual monitoring unit, key lockbox, training associated with the use of the system, periodic checking to ensure that the unit is functioning properly, equipment maintenance calls, response to an emergency call by a medical professional, para-professional or volunteer, and follow up with the older person.
Health & Wellness Initiatives Activities related to the prevention and mitigation of the effects of chronic disease. Provides for the implementation of evidence-based programs to improve health and well-being and reduce disease and injury. Programs at this time include Texercise Select.
Services that include provision of medications, nutritional supplements, glasses, dentures, hearing aids or other devices necessary to promote or maintain the health or safety of the older person. Services provided by trained and supervised homemakers involving the performance of light housekeeping tasks, home management, meal preparation, and shopping assistance.
The objective is to help the older person sustain independent living in a safe and healthful home environment. Service provided through the consumer directed services option whereby an individual provider is chosen by the older person.
Services may include the performance of light housekeeping, meal preparation, escort tasks, and shopping assistance, provided for an older person who requires assistance with these activities in their place of residence. The objective is to help the older person sustain independent living in a safe and healthful home environment.
Provides a one-time-only assistance in the form of a payment to a third-party provider for services or goods that support the basic needs of the person, on behalf of an older person or their caregiver. This could include expenses for basic utility costs, one month’s rent, or supplies necessary to sustain a healthy environment and independent living. All services are dependent upon funding availability and screening criteria.
Older Relatives Caregiver Providing services for Grandparents and other Relatives 55 and older who are raising children under the age of 18. Services include assistance with In order to be eligible for services, biological parents CANNOT be living in the household.

Services that provide or arrange for taking an older person from one location to another.
An older person requests the transportation service in advance of their need. Does not include any other activity.

West Central Texas Ombudsman Program Services to protect the health, safety, welfare, and rights of residents of nursing facilities and assisted living facilities, including identifying, investigating, and resolving complaints that are made by, or on behalf of, residents. Assist those residing in a nursing facility or assisted living home in advocating for their resident rights.
An ombudsman helps ensure that residents get the care they want and the dignity they deserve. Ombudsmen support and promote the health, safety and rights of all residents in long term care. Provide information to individuals and families on topics such as choosing a facility, eligibility criteria and other services for older Texans.

Select “Option 2” when calling. Provides Information And Assistance An ombudsman is a good source of information about selecting a long-term care facility, eligibility criteria, and other services for long-term care residents. Regional programs present facts about facilities.
Residents’ rights are a focus of the program. Advocates With Other Organizations State and regional Ombudsman programs work cooperatively with other advocacy and membership organizations to comment on and recommend legislation and regulatory changes that affect Texas long-term care residents. Staff routinely serves on boards and committees of other organizations and actively advocate for policies to promote quality of care.
The resident has a right to a dignified existence, self-determination and communication and access to persons and services inside and outside the long-term care facility. Each resident must receive and the facility must provide the necessary care and services to attain or maintain the highest practicable physical, mental, and psycho social well- being according to his or her comprehensive assessment and plan of care.
A facility must care for its resident in a manner and an environment that promotes maintenance or enhancement of each resident’s quality of life (respect for individual, participation in groups and activities, and accommodation of needs).
It must use its resources effectively and efficiently to attain or maintain the highest practicable physical, mental, and psycho-social well-being Overview Of The Ombudsman Program The Texas Ombudsman Program advocates for quality of life and quality of care for all residents in long-term care facilities. By state and federal authority, ombudsmen identify, investigate and resolve complaints made by or on behalf of residents.
They provide services to protect health, safety, welfare and rights. In addition, they support the development and operation of family and resident councils. Through twenty-eight area agencies on aging (AAA), certified ombudsmen (staff and specially trained volunteers) serve residents, their families, and friends.
Professional staff supervise the volunteers. Certified volunteer ombudsmen complete a training course, internship, and eighteen (18) hours of continuing education per year. To promote quality of care, the program works with professionals, advocacy and membership organizations that are interested in long-term care and elder rights issues and coordinates with regulatory agencies.

Volunteers who deliver meals to homebound older persons have an important opportunity to check on the welfare of the homebound elderly and are encouraged to report any health or other problems that they may observe during their visits. Congregate Meals differ in that they are served in community settings such as senior centers, churches, or senior housing communities.
These meal sites offer an opportunity to meet new or old friends and engage in social activities while having a nutritious meal. Most sites offer a hot lunch, however some offer evening meals. Come early and play your favorite game (we have several or bring your own) read the newspaper or just visit and have coffee. We have different activities weekly.
Be sure to come by and pick up a copy of our menu and activity calendar. We serve lunch from 12:00 to 12:30 pm Monday-Friday . All of our meals are prepared with love for you right here at the center.
Cost for seniors 60 and older is a suggested donation per meal. For those who are visiting with a senior, the required cost is $6. 44 per person.
