HRSA Rural Health Care Services Outreach Program is sponsored by Health Resources and Services Administration (HRSA). Supports projects that improve access to quality health care services in rural communities.

Rural Health Care Services Outreach Program - Rural Health Information Hub Rural Health Care Services Outreach Program The Federal Office of Rural Health Policy's Rural Health Care Services Outreach Program aims to expand healthcare delivery and address the health needs of rural communities by working with community partners and using evidence-informed models.

In addition to the Regular Track, the program also includes the Healthy Rural Hometown Initiative (HRHI) Track, which focuses on the five leading causes of avoidable death — heart disease, cancer, unintentional injury/substance use disorder, chronic lower respiratory disease, and stroke. A focus on maternal health is also included under this track to address rural needs related to maternal health outcomes.

Breaking the Cycle: Missouri Community Paramedicine Program Brings Primary Care to High-Risk Patients With support from the Outreach Program’s Healthy Rural Hometown Initiative, a partnership between a Federally Qualified Health Center and the local ambulance district provides preventive care to chronically ill patients in their homes.

Pennsylvania Hospital Transitions HRSA-Funded Program to Online Services and Addresses COVID-19 Stress An Outreach Program awardee transitioned its chronic disease program to an online format when the COVID-19 pandemic hit, while also developing new initiatives to address stress in the healthcare workforce and the larger community.

Making a Difference for Generations to Come: School-Based Oral Healthcare in Louisiana A multi-time Outreach Program awardee provides primary care oral health assessment and fluoride varnishes in 12 school-based health centers.

Oregon Washington Health Network and Yellowhawk Tribal Health Center “Tackle Everything” to Improve the Health of Local Community Members Funding from the Outreach Program has supported behavioral health services of the community. Rural Health Care Services Outreach Program Supports Features three Outreach Program awardees working to enhance healthcare access.

Pennsylvania Mobile Team Addresses Substance Use Disorders Support from the Outreach Program allowed this awardee to launch a mobile team to provide support and education to adults and adolescents showing signs of substance use disorders.

Change from the Inside Out: Ohio Wellness Program Inspires Healthy Bodies Through Healthy Minds Multiple Outreach Program grant awards, along with Small Health Care Provider Quality Improvement Program support, have helped develop a wellness program in rural Ohio focused on long-term physical health.
