This project will allow ICFs access to funds to purchase Assistive Technology for the people who live there, based on connections made in the Rapid Response System, or through other consults with Supportive Technology professionals. This funding is specific to the individual assistive-technology needs of people who reside in ICFs. • Use of the Rapid Response System is preferred.
However, DODD will consider applications based on the recommendations of qualified consultants. In the event the Rapid Response system is not used, the name, credentials, and resume of the qualified consultant who determined the appropriate device/technology to support the individual should be provided along with the application.
This documentation should demonstrate how the following criteria, as outlined in the “Ohio Administrative Code Rule 5123-9-12 | Home and community-based services waivers - assistive technology under the individual options, level one, and self-empowered life funding waivers” (https://codes. ohio. gov/ohio-administrative-code/rule-5123-9-12) are met.

FY24 spans February 29, 2024 through June 30, 2024 and FY25 spans July 1, 2024 – March 31, 2025. DODD will award funding to projects that meet the requirements as described in this document until the application deadline or the total available funding for the project is awarded, whichever comes first.
