Quintero, Secretary IDHS Grants Administration Family & Community Services Grants - FY 2026 Office of Family Wellness Opportunities Illinois - Equitable Access Towards Sustainable Systems (IL-EATS) Program (26-444-80-3189) GA - Illinois Equitable Access Towards Sustainable Systems (IL-EATS) (26-444-80-3189) Grant Application Information Eligibility for FY26 Bureau of Family Nutrition (BFN) renewal grants is limited to current providers who are prequalified and in good standing.
Each applicant must continue to be GATA Pre-Qualified and registered in the CSA . Applicant organizations must complete the FY26 ICQ before an award can be made. The ICQ is available in the Illinois GATA Grantee Portal and should be completed as soon as possible.
Applicant organizations must submit a complete FY26 Uniform Application for Grant Assistance Application (pdf) and a Grantee Conflict of Interest Disclosure(pdf) to DHS. OFWContracting@illinois. gov before an award can be made.
Please include CSFA# on the email subject line. All fields on pages 2 and 3 are required. For line #45 for "Amount Requested from the State," please enter FY26 award amount provided by the program administrator.
The Bureau of Family Nutrition reserves the right to request additional information from the applicant to assist in FY26 funding and programmatic decisions. Any changes made to grantee mailing/billing address or Administrator/Executive/CEO name/email should be reported ASAP to DHS to ensure proper information is reflected in the contract documents.
All applicants for Illinois Equitable Access Towards Sustainable Systems (IL-EATS) Program funding, must submit the following document electronically by June 30, 2025. FY26 Uniform Application for Grant Assistance Application (pdf) Once all applications have been received, DHS will determine a contract amount and this will be communicated electronically to the e-mail address that submitted the grant application.
The completed grant application document is to be emailed by July 31, 2025 to the email address DHS. OFWContracting@illinois. gov with the subject line stating: Last 4 digits of CSFA #3189 Provider Organization Name For example: 3189 Jane's DV Agency or 3189 HOPE Agency Note: Everything is being tracked by the CSFA number, so it is important that all required materials and correspondence contain this information.
