Government Organizations; Nonprofit Organizations; Illinois WISEWOMAN Program grants are awarded to IWP Lead Agency grantees, which are required to also administer the Illinois Breast and Cervical Cancer Program grants, since recipients of IWP services must be an IBCCP participant. Participants receiving IWP services must be women who are 35-64 years old, at or below 250% of federal poverty level, who are uninsured or under-insured.

Credentials / Documentation All grantees are required to register with the State of Illinois through the Grant Accountability and Transparency Act (GATA) website, www. grants. illinois.

Upon completion of the application submission and review process, and the provision of any missing or corrected items as appropriate, each successful grantee will receive a grant agreement to be signed by the entity’s authorized official. The grant agreement is not binding on the parties until it has been fully executed by the Illinois Department of Public Health and a fully executed copy has been provided to the grantee.

Range of Approval or Disapproval Time Application approvals will be provided prior to the anticipated Program Start Date of 9/30/25. Not applicable as this is a non-competitive funding opportunity.

Formula Matching Requirements Illinois WISEWOMAN Program grant applicants must submit budget items in accordance with the budget categories detailed in EGrAMS: Program Management, Patient Services (Clinical), and Healthy Behavior Support Options. Indirect costs, research and development, and construction costs are not allowed to be reimbursed by the Illinois WISEWOMAN Program.

Monthly Reimbursement Certification Form and Quarterly Scope of Work Report Grantees are be subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor’s Office of Management and Budget. Deliver risk reduction counseling to 100% of women screened. Risk reduction counseling includes appropriate referral to health coaching, community resources, or lifestyle programs. 2.

Follow up with 100% of women with abnormal blood pressure values (greater than or equal to 120/80). 3. Ensure that 80% of women referred to a lifestyle program or health coaching participate in the program.

Participation is defined as attendance at a minimum of one lifestyle program or coaching session. 4. Ensure that 60% of women who participate in a lifestyle program or health coaching complete the program.
