Implementation Partner RFI: Large-Scale Health Care Initiative is sponsored by Heartland Whole Health Institute. Heartland Whole Health Institute is seeking implementation partners for a five-year pilot project designed to transform how chronic cardiometabolic diseases—such as diabetes, hypertension, obesity, and high cholesterol—are prevented, managed, and reversed. Through research, advocacy, and education, we develop care and payment models that can be duplicated here in the Heartland and across the country.
We also work with policy leaders and government officials to influence policy that support health care for all. Health Care Transformation Demonstration Project: Transforming Health Care in Arkansas Chronic diseases profoundly impact individuals and communities across Arkansas, diminishing quality of life, limiting daily activities, and, tragically, leading to premature death for countless residents.
This burden is especially heavy in rural areas, where people often struggle with fragmented access to needed services and rising health care costs. Heartland Whole Health Institute’s Arkansas Health Care Transformation Demonstration Project is designed to ignite a profound change for the Arkansas community and fundamentally reshape health care delivery across the state.
This pioneering five-year Project aims to empower Arkansans at risk of or currently living with cardiometabolic conditions like diabetes, hypertension, and obesity to reclaim their health and well-being. The Project will evaluate how coordinated clinical, behavioral, and social interventions can reverse the trajectory of these diseases, with the goal of proving that this approach improves health outcomes and reduces the costs of care.
This Project goes beyond traditional health care — it’s about transforming lives, ensuring that proactive prevention replaces reactive treatment, and that every Arkansan can achieve their healthiest future. Our vision is to create a state where health is no longer dictated by ZIP codes, and thriving communities benefit from accessible, high-quality care, truly making lives better for generations to come.

Vision 2030: Roadmap for Health Care Transformation In November 2024, the Northwest Arkansas Council and Heartland Whole Health Institute released “Northwest Arkansas Health Care Vision 2030: Continuing the Transformation,” a report conducted by Tripp Umbach , outlining progress and future goals to transform health care in the region.
Since the creation of the Health Care Transformation Division in 2019, Northwest Arkansas achieved significant milestones, including reducing patient outmigration from nearly $1 billion in 2018 to $695 million in 2023. Investments totaling $529. 5 million have laid the foundation for future growth.
The health care sector has become a major economic driver, with its gross regional product (GRP) increasing by 80% from 2018 to 2023, outpacing the overall regional economic growth of 37%. Estimated projections indicate the sector could reach $6. 1 billion by 2030, representing a 144% growth rate, according to Tripp Umbach.
Given that Washington and Benton counties surpassed 590,000 residents in 2024 and estimated projections show a population of over 1 million by 2050, the strain on healthcare services will continue to grow.
The Vision 2030 report outlines a comprehensive roadmap to strengthen Northwest Arkansas’ health care system, ensuring it meets the needs of a rapidly growing population while positioning the region as a national model for excellence in health care delivery.
Morehead-Cain Foundation Host Organization Since 2023, Heartland Whole Health Institute has served as a host organization for the Morehead-Cain Foundation Civic Collaboration program.
