Living Donor Assistance Program (Georgia residents) is sponsored by Georgia Transplant Foundation. Provides financial support to living kidney and liver donors in Georgia to help with loss of wages during leave of absence for donor surgery, travel expenses, childcare support, and other related expenses resulting from their organ donation.

The Living Donor Assistance Program provides financial support to living kidney and liver donors as determined by their financial situation and the hardships created as a result of their organ donation. I am so grateful for your help. Without financial assistance from the Georgia Transplant Foundation, I would not have been able to donate my kidney.

Some living donors face difficult financial situations that involve loss of wages during leave of absence for donor surgery, travel expenses, childcare support and other related expenses.

**The goal of the Living Donor Assistance Program is to provide financial support to living kidney or liver donors as determined by their financial situation and the hardships created as a result of their donation. ** * Either the living donor or the transplant recipient must be a resident of Georgia. * Assistance is available for hardship expenses directly created by the donation.

* Applications are to be submitted prior to the treatment. * The living donor is expected to use all available sick days, but may retain up to two weeks of vacation time. * Living donor applications are expected to show evidence of the patient/family’s support and financial planning for the surgery and recovery time.

### Eligibility Guidelines: * Georgia resident (at least one person from the donation chain must be a resident of the state). * Solid organ transplant. * Proof of financial need.

* Evidence of working on a permanent solution to address the need. * Show a transplant-related need. 1.

Discuss your need with your transplant center social worker. 2. Submit completed application to your social worker or through the online application form below.

3. Provide documents to support the requested assistance. 4.

Applications will be reviewed, once submitted and complete. 5. Requesting social worker will be notified regarding the review decision.
