Local Indigent Care Needs - CMSP Local Indigent Care Needs Healthcare Workforce Grant Healthcare Infrastructure Grant Local Indigent Care Needs Grant The Local Indigent Care Needs (LICN) grant program was created by the CMSP Governing Board to expand the delivery of locally directed indigent care services for low-income uninsured and under-insured adults who lack access to health, behavioral health and associated support services in CMSP counties.

The primary objectives of the LICN program are to promote timely delivery of necessary services to locally identified target populations, to link those populations to other community resources and support, and to improve their overall health outcomes. Awards were granted across two funding tracks.

Track One – Planning Project Grants Award amounts ranged up to $50,000 per Planning project (up to $75,000 if the project served more than one CMSP county). LICN Planning project grants were awarded to CMSP county or non-profit agency applicants who demonstrated capacity to bring local stakeholders together to identify local indigent care needs and apply for an associated Implementation Program grant.

CMSP awarded 23 LICN Planning project grants a collective award total of $1,144,846. Awarded Planning projects served 21 CMSP counties. Track Two – Implementation Program Grants Award amounts ranged up to $1,500,000 per Implementation project (up to $500,000 per year).

LICN Implementation Program grants were awarded to CMSP county and non-profit applicants to meet locally identified indigent care needs across the following four focus areas: Local Level Care Management – Developing care management interventions that are tailored to meet individual client needs, that connect clients to other services and support, and that comprehensively document and track client service provision.

Continuity of Care – Creating county-wide or regional care strategies that facilitate linkages across the continuum of care including inpatient care to respite, access to specialty care, primary care, prescription medical support, mental health treatment, and substance use treatment.

Enabling Services – Assisting clients in obtaining nutritional support, housing, transportation, legal assistance, and income assistance and/or providing access to services not otherwise available in the community through new service creation or expansion of currently limited services.

Disease Management – Halting or decreasing the severity of the conditions of clients with chronic, health or behavioral health conditions through strategies including symptom management, medication compliance, adherence to treatment plans, education and/or lifestyle changes. CMSP awarded 31 LICN Implementation Program grants a collective award total of $42,575,024. Awarded Implementation projects are serving 26 CMSP counties.
