Eligible organizations include primary care practices, Federally Qualified Health Centers, and organizations prepared to sponsor an advanced primary care practice The MHCC is seeking applications from eligible organizations, including primary care practices, Federally Qualified Health Centers, and organizations prepared to sponsor an advanced primary care practice.
To be considered, applicants must demonstrate a clear understanding of community health needs, readiness to establish or expand primary care services, and the capacity to build sustainable operations.
