Pride Center's client population is diverse sexual and gender minority (SGM) individuals affected by adverse Social Determinants of Health (SDOH), with emphasis on African American SGM who face high rates of health disparities as well as adverse and disproportionate impacts from COVID-19.
Grantee Organization: Grassroots Crisis Intervention Center Jurisdiction Served: Multiple Counties Grant and Amount: 23-16 ($500,000) The program will add 24-hour behavioral health urgent care services to Grassroots' existing SUD crisis stabilization services in Central Maryland, focusing on individuals who are low-income, African American, and/or women.
Grant funding will allow Grassroots Crisis Intervention Center to reduce barriers to accessing affordable, quality urgent behavioral health care; decrease the acuity of behavioral health symptoms; and decrease hospital emergency department visits for behavioral health patients.

Grantee Organization: Medstar Franklin Square Hospital Jurisdiction Served: Baltimore County Grant and Amount: 23-01 ($360,000) The program seeks to address disparities in chronic disease and food access, as well as other disparities faced by African American residents in Baltimore City and Baltimore County.
MedStar Franklin Square Hospital will provide healthy food for patients to prepare nutritious meals for their families, connect patients to a dietician and Community Health Advocate, and provide 20 hours of health education. Through the Primary Care Center and Family Health Center, patients will have access to primary care, specialty care, prevention services, and behavioral health assistance.
Grantee Organization: Sinai Hospital Jurisdiction Served: Baltimore City Grant and Amount: 22-01 ($360,000) The project seeks to address disproportionately high rates of diabetes, heart disease, and hypertension, and the disproportionate impact of COVID-19 among Black residents in northwest Baltimore.
Two Community Health Workers (CHWs) will be hired to work at the LifeBridge Center for Hope and the Sinai Hospital Addictions Recovery Program (SHARP) and support the mobile health unit. CHWs and peer recovery specialists will screen individuals for somatic and behavioral conditions, provide linkages to care and wraparound services, and deliver case management including one-on-one wellness planning.
Grantee Organization: Care for Your Health Jurisdiction Served: Prince George's, Howard, and Montgomery Counties Grant and Amount: 22-07 ($225,000) This project will expand access to culturally and linguistically competent grief counseling among low income and elderly communities of color. Key interventions include the integration of bilingual grief counseling into the applicant's primary care services.
Grant funds will be used for salaries for a Grief Counselor, Program Manager, and Primary Care Physician. Grantee Organization: Helping Up Mission Jurisdiction Served: Baltimore City Grant and Amount: 22-09 ($285,000) This project seeks to expand access to Substance Use Disorder (SUD) and mental health services among individuals experiencing homelessness in Baltimore City and surrounding areas.
Key interventions include use of a converted school bus and intake van to conduct street outreach for residential SUD programs, casework, and other referrals.

Key interventions include the opening of a new facility to provide primary care, counseling, and other Behavioral Health services; law enforcement referrals and ED diversions; case management to connect clients to partner organizations addressing SDOH; and respite care post-hospital discharge.
Target outcomes include a reduction in ED admissions for chronic conditions and mental health and substance use disorders, increased access to primary and preventative care, decreased recidivism in the criminal justice system, and a reduction in overdoses.

The project will provide on-site access to dietary and diabetic educators, healthcare navigation, wrap-around case management, and job training services; walk-in testing for diabetes (HgA1c) and retinal neuropathy; and referrals to mental health services. The goal is to decrease the rate of ED utilization for ambulatory care sensitive conditions, improve management of diabetes, and help reduce the rate of new diabetes diagnoses.

Tablets provide clients with virtual access to: daily psychiatric rehabilitation services; primary care and other healthcare services via telehealth; other virtual rehabilitative services such as music therapy; a Google Classroom with mental health rehabilitation resources and content; and the Sheppard Pratt YouTube platform.

org Grantee Organization: Pressley Ridge Jurisdiction Served: Baltimore County Grant and Amount: 20-012 ($420,000) Grant Project Description: This program will support the HOMEBUILDERS model, which is an evidence-based family preservation program that serves impacted by the opioid crisis who are referred by Child Protective Services, with specific focus on infants and children at serious risk for removal from the home.
The program provides intensive in-home services to vulnerable families with complex health and social service needs over a 28-day period, and referrals for specialized addiction services outside the home.

This program will support an array of services to individuals with SUD treatment needs in this rural and underserved area of the State, including case management, individual and group therapy, assessment and skills development.
The proposal indicates that over the last five years in Garrett County, 47% of children with a parent having an SUD issue have been removed from the home, and the Garrett County Health Department has reported approximately 49 new diagnoses each year.

gov Grantee Organization: Behavioral Health Leadership Institute Jurisdiction Served: Baltimore City Grant and Amount: 20-017 ($220,000) Grant Project Description: This program will expand the Behavioral Health Leadership Institute's 'pop up' clinic program that uses a mobile van to provide access to low-threshold medication for opioid use disorder to vulnerable residents at two new sites in Baltimore City that have requested these services.
The target population includes justice-involved residents who are shunned from traditional SUD programs and homeless individuals impacted by multiple barriers to accessing health care services. The program currently provides services a half-day per week per site, at three sites in Baltimore City.

The project would develop and implement a systematic prevention, early detection, and referral program to prevent substance use. CHRC grant funds would support a contract with the Center for School Mental Health at University of Maryland School of Medicine and mini-grants to substance use treatment agencies selected via RFP to provide Certified Addiction Counselors at participating schools at least one day per week.
The program is designed to be implemented in all high schools, and other support services staff (school counselors and social workers) would be trained on the use of the CRAFFT 2 screening tool and the brief negotiated interview. This project involves two new programs: (1) a 14-bed Joint Commission on Accreditation of Healthcare Organizations accredited transitional recovery housing program in partnership with Johns Hopkins Hospital Broadway Center for Addictions; and (2) a 16-bed long-term Spiritual Recovery Program that focuses on integrated modalities of care, focusing on hope, health, housing, and a sense of purpose and community.
Both of these programs are modeled after success in serving adult males. The project would integrate a primary care opioid treatment program as part of the existing Behavioral Health Program, expanding services to include Medication-Assisted Therapy (MAT) for opioid use disorders, routine and special diagnostic procedures and toxicology tests, psychiatric/psychological assessments and treatment through the clinic’s Behavioral Health Program.

org/ Grantee Organization: Associated Catholic Charities Jurisdiction Served: Multiple Grant and Amount: 19-015 ($510,000) Grant Project Description: This project would enhance and expand Catholic Charities’ substance use treatment program to provide Integrated Dual Disorder Treatment (IDDT).
The project would enable the organization to become IDDT certified and would support policy, procedure, and system of care changes, capacity building, and staff training. The ability to provide expanded, integrated substance use treatment could enhance the service offerings for as many as 1,500 clients of Catholic Charities programs and the Villa Maria Community Resources Clinics per year.

org/ Grantee Organization: Cecil County health Department Jurisdiction Served: Cecil County Grant and Amount: 19-016 ($600,000) Grant Project Description: This project would provide services to low-income pretrial detainees involved with the Office of the Public Defender on misdemeanor or nonviolent felony charges who have a substance use disorder and lack access to appropriate treatment services in the community.

org/ Grantee Organization: Cornerstone Montgomery Jurisdiction Served: Montgomery County Grant and Amount: 19-017 ($325,000) Grant Project Description: This project would expand access to somatic and preventative care services to an estimated 565 individuals who are living in poverty, medically frail, experience comorbid health and behavioral health disorders, and are senior citizens.
The applicant would collaborate with Nexus Montgomery, a consortium of six hospitals in Montgomery County, to review data quarterly and identify individuals in need of services. The new Walk-in/Urgent Care Center would provide 24-hour access to behavioral, mental, and addiction services.
The project would provide an array of services, including a 24-hour crisis hotline; outpatient mental health; SUD treatment and MAT services; residential crisis beds; a mobile crisis team; and an ACT team for individuals with serious mental illness, including referrals to community providers. Individuals would be able to access a Peer Recovery Specialist who would perform an in-person follow-up visit. The project would also provide telehealth services, which would provide Screening, Brief Intervention and Referral to Treatment (SBIRT) for disadvantaged populations in Queen Anne’s County. aspx Grantee Organization: Anne Arundel Medical Center Jurisdiction Served: Anne Arundel County Grant and Amount: 19-024 ($322,000) Grant Project Description: This project supports Wellness Connections, a collaborative, integrated care program for individuals seen at Anne Arundel Medical Center (AAMC) with untreated/undertreated mental health needs and substance use disorders in partnership with Arundel Lodge.
The project will serve individuals with co-occuring disorders who are best served through integrated treatment which allows practitioners to address mental and substance use disorders at the same time, often lowering costs and creating better outcomes.
Vulnerable patients with both medical and mental health needs will see greater continuity of care at AAMC and the community as they will be receiving tailored services with the right level of care.

Jurisdiction Served: Baltimore County Grant and Amount: 19-025 ($59,000) Grant Project Description: This project supports the TOGETHER Project which provides counseling and continuing care services to children and adults affected by the opioid crisis. The project seeks to keep families intact and encourage self-efficacy and resilience.
In addition to specialized group therapy, participants receive wraparound social services through the partnership with Catonsville Emergency Assistance to supply links and resources to clients facing eviction, foreclosure, utility cut-offs, and food security. The project will increase knowledge about addiction among affected family members and awareness of the opioid crisis in the general public.

Grantee Organization: Potomac Healthcare Foundation Jurisdiction Served: Baltimore City Grant and Amount: 18-009 ($210,000) Grant Project Description: This project would establish a 16-bed specialty recovery house in West Baltimore for underserved, low-income young adults with opioid addiction to provide a supervised, developmentally appropriate recovery environment with support for outpatient treatment.
The project would provide an immediate linkage to care from the ED or hospitalization with a 'warm handoff' and rapid initiation of treatment, including residential recovery support and buprenorphine as needed. While at the center, patients would benefit from an on-campus community treatment facility which would provide a full spectrum of behavioral health treatment for opioid addiction and co-occurring disorders.

org Grantee Organization: Wells House Jurisdiction Served: Washington Grant and Amount: 18-010 ($103,615) Grant Project Description: This project would add somatic care services at two addiction treatment facilities in Western Maryland.
Many of the patients at Wells House have complex medical needs, and providing integrated behavioral and somatic care could lead to a reduction in avoidable hospital utilization for this vulnerable population. The project would utilize a nurse practitioner and medical assistant to perform health assessments, provide necessary health education, and address other somatic health issues.

org Grantee Organization: Atlantic General Hospital Jurisdiction Served: Lower Shore Grant and Amount: 18-011 ($175,000) Grant Project Description: This project would develop a new interdisciplinary chronic pain management center that would provide access to somatic health, behavioral health, and therapy services to help patients relieve chronic pain without the use of opioid medications.
For those with Substance Use Disorders, the intervention offers a concentrated outpatient program using a multi-disciplinary approach to reduce or discontinue the use of opioids for pain management.
