Medical Assistance Grants - Vitalyst Health Medical Assistance Grants Vitalyst Health Foundation was established after the sale of the St. Luke’s Hospital System in 1995. In keeping with the hospital’s tradition of charity care, restricted funds for Medical Assistance Grants were transferred to the foundation.
Every three years, Vitalyst invites proposals from nonprofit or fiscally sponsored organizations that provide services and treatment related to hearing, vision, substance use disorder, and heart and lung health. Medical Assistance Grant applications are due April 16, 2026, at 5:00 P. M.
through the grant portal. These grants support direct services that improve access to care and quality of care for low-income and underserved residents of Maricopa County. Funds may be used for screenings, training, surgeries, program development tied to direct services, and client-related equipment such as assistive and adaptive devices.
Nonprofit or fiscally sponsored organizations Directly serving low-income or underserved individuals, measured by: enrollment in a government assistance program (SNAP, AHCCCS, KidsCare, WIC, Free/Reduced School Meals, etc.) or annual household income less than 250% of federal poverty guidelines, in 2026 for Arizona set as $39,900 for an individual income or $82,500 for a family of four ( https://aspe. hhs.

Services provided to individuals in Maricopa County Maintenance of a log recording direct services provided to individuals Submission of a mid-year and annual report documenting individuals served Hearing: up to $150,000 per year for three years ($375,000 total available) Vision: up to $150,000 per year for three years ($350,000 total available) Substance Use Disorder: one grant for $25,000 per year for three years ($25,000 total available) Heart and Lung: up to $100,000 per year for three years ($230,000 total available) Vitalyst requests proposals from nonprofits or fiscally sponsored organizations that provide services, treatment, and programs in the areas of hearing, vision, substance use disorder, and heart and lung that leverage existing resources and maximize community benefit.

com/vitalyst . Awards are announced on Friday, June 26, 2026. The grant period is July 1, 2026 – June 30, 2029.
Proposals should include: Legal name and address of the organization Proposal narrative within portal field including: Direct Service (40 points; 4,000 character count) : a description of the medical assistance to be provided relating to hearing, vision, substance use disorder, or heart and lung.
For efforts that provide and support direct services, including but not limited to screenings, evaluations, diagnostic testing, exams, and surgeries; client-related equipment such as assistive devices and adaptive equipment; and service-related program management/administration, such as provider expenses/vouchers/referrals, program staff compensation and benefits, client transportation, and interpretive/translational services.
Population(s) Served (40 points; 4,000 character count) : the population of focus and geographic region to be served, including the number of individuals expected to be served or impacted by the service.
