Through scaled, multi-year funding, the Community Fund supports organizations tackling complex challenges that contribute to health disparities in the communities we serve. By investing in programs that address social determinants of health, NewYork-Presbyterian partners with CBOs to advance equity, strengthen community infrastructure, and create conditions that support long-term health and stability.
The Community Fund is directly informed by findings from NewYork-Presbyterian’s Community Health Needs Assessment (CHNA) , a comprehensive process that brings together community voice, data analysis, and stakeholder insight. The CHNA helps identify priority issues and neighborhoods where deeper, sustained investments can make a meaningful difference.
For the 2026 funding cycle, the Community Fund will prioritize programs addressing either housing or food insecurity , two critical needs identified through the most recent Community Health Needs Assessment. Community Fund grants are designed to support organizations with the capacity to implement and sustain complex initiatives that improve stability, access to resources, and overall community health.
Grant Size: $75,000–$150,000 per year Grant Term: Up to two years To be considered for funding, organizations must: Be a not-for-profit, religious, or government agency with a clear community service mission Have a minimum annual operating budget of $1M Propose a project or program that addresses one of the priority social determinants of health identified through the CHNA* For more details and information about the Community Fund, click here .
