Many of the diseases and conditions covered by the NIAMS mission areas cause pain, disability, or disfigurement. Some of these diseases and conditions are very common, while some are rare, affecting only a few thousand people worldwide. Pain, both acute and chronic, is one of the most important symptoms affecting mobility and quality of life for patients.

Yet, the mechanisms of pain, and measures and treatment options that can prevent and alleviate pain caused by these diseases are not clearly understood and remain a critical knowledge gap within the NIAMS mission.

This NOSI invites research applications that focus on improving the understanding of mechanisms of acute and chronic pain, and enhancing knowledge that will help better prevent, manage and reduce pain and improve health equity of all populations suffering from acute or chronic pain caused by rheumatic, autoinflammatory, musculoskeletal, and skin diseases and conditions within the NIAMS mission.

Examples of research topics include, but are not limited to: Understanding the role of pain in acute and chronic rheumatic, musculoskeletal and skin diseases/conditions and its effect on key outcomes. Understanding the mechanism of pain, susceptibility, transition from acute to chronic pain, phenotypic manifestations, heterogeneity in rheumatic, musculoskeletal and/or skin disease from a biopsychosocial perspective.

Determining the relationship between tissue damage and the experience of pain. Identifying functional outcome measures and biomarkers for pain that could be used to assess the effects of treatments. Developing and exploring the use of appropriate animal models of acute and chronic pain to better understand its pathophysiology and etiology and to explore treatment options.

Exploring and validating both existing and new tools that will allow for accurate diagnosis and optimized, personalized treatment of chronic pain conditions. Leverage existing dataset for secondary analysis and data reuse for pain research.

Making large rheumatic, musculoskeletal and/or skin disease data sets relevant to pain research FAIR (Findable, Accessible, Interoperable and Reusable) Understanding the relationships between pain and itch. Strategies for managing pain post orthopaedic surgery or injury, reducing acute pain and preventing transition from acute to chronic pain.

Characterizing reversible and permanent biochemical, inflammatory, autoimmune, and anatomic changes that cause or are caused by chronic pain. Developing novel therapeutic approaches to treat chronic pain including non-opioid-based analgesic drugs, personalized therapeutics, behavioral interventions, and physical activity to improve both pain and related comorbidities such as fatigue, depression, and sleep disturbance.

Using interdisciplinary approaches to address pain as a multisystemic problem, integrating and bridging research from multiple fields. Developing and employing strategies/interventions to reduce health inequities in pain experience and management encountered by members of underrepresented groups with rheumatic, musculoskeletal and/or skin diseases and conditions.

Application and Submission Information This notice applies to due dates on or after November 5, 2021 and subsequent receipt dates through January 10, 2023.
