KANSAS DEPARTMENT OF HEALTH AND ENVIRONMENT (KDHE) REQUEST FOR PROPOSAL (RFP) FISCAL YEAR 2025 MINI -GRANT # Older Adult Falls Prevention and Chronic Disease Self -Management The Kansas Department of Health and Environment (KDHE) invites regional and community -based organizations to apply for mini -grants, not to exceed $ 4,000 .

These mini -grants may be used for increasing the number of Trainers, and the number of Master Trainers that teach the class or to support a current trainer in an established class setting with more than six participants .

Designed to develop capacity for, bring to scale, and sustain evidence -based falls prevention and self -management education programs that empower older adults and adults with disabilities to better manage chronic conditions and promote falls prevention. 1. Support community -based initiatives .

2. Assist community -based programs in the implementation of initiatives that address changes in knowledge, attitudes, behaviors, and/or environments. 3.

Evaluate intervention effectiveness by tracking: Complete Pre and Post Program Surveys (Submit within 1 week of the workshop Number of local organizations involved. Identification of target populations. Details of implemented interventions or educational efforts.

Number of individuals trained and participating in the program. 4. Create a business plan with sustainability in mind; include program operations, organization capacity, organizational goals and vision statement.

# Eligibility and Funding Private or public Non -profit community -based organizations, units of local government, state government agencies , and Area Agencies on Aging are eligible. Only one application per community will be reviewed. The grant opportunity extends until May 31, 2026.

Funds must be used for the development , coordination, implementation, and evaluation of community -based interventions . To allow for a falls prevention or self -management support program, increase the skills or behaviors relevant to chronic disease self - management , and reduce the risk of falls among older adults .

This is a competitive mini -grant process, with awards up to $4,000 per community, contingent on the availability of federal funds. What Are Evidence -Based Programs? 3.

Chronic Disease Self -Managemen t Program (CDSMP) 4. Diabetes Self -Management Program (DSMP) 5. Chronic Pain Self -Management Program (CPSMP) Applicants must commit to hosting a minimum of two trainings, with a minimum of six participants.

Applicant must submit a project timeline and budget with their application. A final project report and expenditure affidavit must be submitted within 30 days of the grant period’s conclusion. Pre - and post -surveys must be submitted following the completion of each workshop.
