PAN Foundation Transportation Assistance Fund

PAN Foundation Transportation Assistance Fund is sponsored by PAN Foundation. Provides financial support for transportation to and from activities that improve health outcomes for individuals with life-threatening, chronic, and rare diseases.

Transportation financial assistance for medical and health activities Beginning June 26 at 5:30pm ET, PAN’s call center and portals will be closed as we transition to the new unified TotalAssist program . We encourage you to complete any enrollments, grant management tasks, or time-sensitive claim submissions prior to June 26. (Note, our contact center will also be closed on Friday, June 19 in observance of Juneteenth.)

Our new call center and TotalAssist portals will be available again starting July 1 at 8:30am ET. All active and recently expired (January 1, 2026, or later) PAN legacy grant information will be available again on July 1 via the TotalAssist portal. The new portal can be found at TotalAssist.

The transportation fund provides financial support to PAN patients with life-threatening, chronic, and rare diseases to access affordable and reliable transportation to and from activities that improve their overall health outcomes, including healthcare services, social connection and support, and healthy and nutritious food.

If this fund is open , and you are eligible, apply today. If this fund is closed , join the wait list. If a fund is open to wait list only , people currently on the wait list can apply using the link sent by email.

Before you apply, make sure you have this information ready: Your diagnosis and medications Your provider’s contact information, including name, and phone number Your health insurance carrier name and your insurance member ID Your contact information, including email and social security number Your adjusted gross annual household income Healthcare providers and pharmacies will be asked for additional information.
