Healthcare providers and pharmacies can check eligibility and apply on behalf of their patients. There are two ways to apply for a new grant when a fund is open: Apply online 24/7 through the PAN portal . Apply by phone: call 1-866-316-7263 Monday through Friday, 9 a.

Read our step-by-step guide to applying in the portal Applying from a wait list Start by checking the email notification If you have signed up for a fund’s wait list, you will receive an email notifying you when PAN has opened your fund to the wait list. That email will contain the patient’s name, a unique wait list ID number, and a unique link to the PAN portal .
The ID number and link are unique to each person and cannot be used for other patients. Click on the unique link in your notification email to begin your application online. If you don’t have a PAN portal account yet, you can create one at that point and continue with your application.
You will have two business days to apply online using the unique link in the email, or by phone. After two business days, the email link will expire. If you do not apply within those two business days, you will be removed from the wait list.
Two ways to apply from a wait list Apply online 24/7 in the PAN portal: In the email notification you received, click the unique link to be directed to our online portal to apply. You will have the chance to create your PAN portal account at that step. Apply by phone: 1-866-316-7263, Monday through Friday, 9:00 a.

ET. In the email you received, you will receive a wait list ID number . When you call us to apply, please have the wait list ID number at the ready and a representative will help you through the application process.

Learn how to enroll your patients Verification after you apply Sometimes, we may conditionally approve your application and will request that you verify your income or provide documentation of your health insurance premium in order to provide final approval.
If you have been asked to verify your income, use our online income verification form to submit documents for income verification. Have your 1040 tax form ready.
If you do not have a 1040 tax form, you will need to provide the following information: Brief explanation of the reason you don’t have the 1040 tax form Verify your health insurance premium If you have been asked to verify your insurance information or health insurance premium amount, you can use our online premium verification form to submit documents for health insurance premium verification. Have your insurance information ready.
You will need to attach a digital copy (such as a photo or a PDF) of one (1) of the following: Social Security Administration benefit verification letter Paystub(s) for one whole month Insurance statement/bill from educational institutions Verify your health insurance premium After your grant is approved Once your grant is approved, you can use your PAN grant funds immediately.
Just give your enrollment information to your provider or pharmacy. You’ll get an official welcome letter and PAN identification card via email or in the mail within five to seven business days. m. ET. Activate your debit card (transportation only) If you were approved for a transportation grant , you will receive a prepaid Visa debit card in the mail 15-20 business days after your grant is approved.
