The Patriots’ Landing houses are located on West St.
in Rocky Hill. There are five separate single family three (3) bedroom homes that can accommodate Veterans, their spouses and children. All houses are completely furnished; equipped with kitchen supplies, bedding, furniture, washers and dryers, and full kitchens.
All houses are smoke free and pets are not permitted (an exception may be considered for verified service animals). • Housing will be provided on a month to month basis, not to exceed twelve (12) months. Unless extended by the DVA for not more than two six month periods based on specified extenuating circumstances.
• Participants are expected to pay a monthly housing Program Fee. • Participants are expected to comply with all terms and conditions listed in the Patriots’ Landing Temporary Housing Program Agreement, which will be provided upon admission.
Through a collaborative effort between the Connecticut Department of Veterans Affairs and Chrysalis Center Inc., a case manager will be assigned to assist couples and families with individual recovery plans to return them to independent living in the community. Case managers will also offer career and educational assistance as well as resume writing, networking and interviewing skills.

Documents required with your application are the following: • Copy of your DD Form 214 • Proof of Connecticut (CT) Residency - Applicants must be official residents of the State of Connecticut. If you DD214 does not indicate that you deployed from or returned to a Connecticut address, please attach a copy of your Connecticut driver's license.
• Health and legal documents may be requested. Copy of Medical/ Health Insurance Cards (VA CT Health Card, Medicare, Medicaid) • An interview will be conducted by CT DVA and Chrysalis Center Inc. staff. Based on that interview, the veteran may be required to provide additional medical, behavioral health or substance abuse information.
• Veterans and their family will be assigned Case Manager from Chrysalis Center Inc. and will be expected to develop and follow an Individual Recovery Plan (IRP) outlining the goals needing to be accomplished to obtain permanent subsidized, affordable or independent housing. When you have completed you application please mail to the Patriots' Landing Program at the Department of Veterans' Affairs.

The SCI Youth Grant Pitch Contest is a competitive program from Social Capital Inc. that funds youth-led community improvement projects in Greater Boston.
