Model-to-Clinic is the translation arm of PRIMED-AI and sits one step downstream of the companion D2M-AIP announcement (RFA-RM-27-012). Where D2M-AIP funds the creation of AI-enabled multimodal clinical decision support tools, M2C funds the harder and less glamorous work of taking a tool that already exists as a validated prototype and getting it to function in real clinical settings - which means confronting workflow integration, site-to-site generalizability, clinician trust, regulatory pathway and sustainability all at once. The distinction matters for applicants because the two announcements share a deadline and a mechanism but reward very different evidence. A team with promising model performance and no deployment experience should apply to D2M-AIP; a team applying to M2C needs to arrive with a validated prototype and a credible clinical partner already in place. The UH3 cap here is $1,000,000 in direct costs per year, higher than D2M-AIP's $800,000, reflecting the cost of clinical implementation work. NIMH administers this announcement on behalf of the Common Fund, though the scope is not limited to mental health - PRIMED-AI as a whole targets significant health challenges broadly, with clinical imaging integrated with multimodal health data as the unifying technical thread. Awards are made under a phased UG3/UH3 cooperative agreement, so milestone definition governs whether a project advances past the exploratory phase. Successful tools developed under this and the other PRIMED-AI development announcements will be independently assessed by the program's Validation Center (RFA-RM-27-014), which applicants should anticipate in how they document performance and reproducibility. Foreign organizations are eligible, and the emphasis throughout is on tools that are not merely accurate but reliable, cost-effective and sustainable in routine care.
