PATH services provide outreach and engage people not currently connected to mainstream mental health services, primary health care, and substance abuse service systems.
Awarded Grantees shall meet the Substance Abuse and Mental Health Services Administration (SAMHSA) goal for PATH programs, which is to reduce or eliminate homelessness for individuals with serious mental illnesses or co-occurring substance use disorders who are homeless or at imminent risk of becoming homeless.
