Wyoming contracts with four community providers to assist people who are experiencing homelessness and have a recent diagnosis of SMI, SED, and/or a co-occurring disorder.

Intent of the Program : PATH’s goal is to assist individuals who are at imminent risk or are experiencing homelessness and have a serious mental illness or co-occurring substance use disorder(s). The services provided link the individuals in need to mainstream and other supportive services to help secure safe and stable housing, improve health, and overall encourage a self-directed, purposeful life.
These funds are not intended to be solely used for housing support (i. e,. rent, safety deposit, etc).
Who can apply : Only Wyoming non-profit organizations, local or county governments, and tribal entities may apply. Applicants must provide any updates to the entity’s Unique Entity Identifier (UEI) number and Certificate of Good Standing from the Wyoming Secretary of State, or any updated letter from the County stating governmental status is required. Please feel free to send this on to any other Wyoming organization that may qualify.
