Provider/Community Grant Program is sponsored by PA Health & Wellness. This program strengthens community-based organizations working to address social and environmental factors that influence health across the Commonwealth of Pennsylvania.

Provider/Community Grant Program Provider/Community Grant Program The PA Health and Wellness (PHW) Grant Program is designed to strengthen community-based organizations working to address social and environmental factors that influence health across the Commonwealth of Pennsylvania. PHW entered the Pennsylvania healthcare market in 2018, and since that time we have strived to be more than a community partner.

We have worked closely with local organizations, providers, and advocates to improve health outcomes across the Commonwealth.

Through the partnership, the program aims to: Expand access to essential community services Strengthen organizational capacity among local nonprofits Promote equitable health, economic stability, and community wellness Create a measurable and sustainable community impact Success will be reflected through the reach of funded programs, quality of services delivered, and demonstrated impact within historically underserved communities.

Proposals should clearly describe how these elements will be achieved and measured. PA Health and Wellness in partnership with the Centene Foundation will award up to $100,000 in grant opportunities.

These will the key drivers for funding: Consideration is given to requests that address: Health Access and Stability – conditions, systems, and supports that enable individuals and families to consistently obtain and maintain the resources necessary for good physical, mental, and behavioral health over time.

Economic Opportunity – ability of individuals and families to access the financial resources, affordable housing opportunities, education, and supportive systems necessary to achieve long-term economic stability and improved health outcomes.

Workforce Development and Education – programs, pathways, and support systems that help individuals gain the skills, credentials, experience, and employment opportunities needed to secure stable, quality employment and achieve long-term economic health and stability.

Community Wellness and Support – the social, environmental, and community conditions that help individuals and families experience safety, connection, stability, and overall well-being.

Giving Requirements and Funding Criteria: All contributions will be made to qualified, federal tax-exempt 501(c)(3) nonprofit organizations, as defined by the Internal Revenue Code Restricted to nonprofit organizations serving residents in areas PA Health and Wellness has a presence. This will include community-based organization across the Commonwealth.

Individual person(s), families, and other private foundations Political, labor, and fraternal organizations For-profit publications or organizations seeking advertisements or promotional support Religious organizations (i.e., churches or organizations tied to place or worship) as determined at the sole discretion of PHW.

Sponsorships of local groups/individuals to participate in regional, national, or international competitions, conferences, or similar events Organizations that discriminate based on age, race, color, religion, national origin, disability, sex, sexual orientation, gender identity, family status and receipt of public assistance, military and veteran status or other legally protected status are prohibited from receiving funding.
