Indiana's GROW (Growing Rural Opportunities for Well-being) initiative, part of the federal Rural Health Transformation Program, offers competitive regional grants to transform rural healthcare.
The model encourages collaboration among hospitals, FQHCs, mental health providers, community-based organizations, and other partners to strengthen healthcare delivery.
