Assisted Living Facility Modernization Grant Turn on more accessible mode Turn off more accessible mode Assisted Living Facility Modernization Grant What would you like to do? The RHTP Assisted Living Facility Modernization Grant is specifically designed to support the modernization of physical environments that provide specialized Memory Care, Dementia Care, and Alzheimer's Care services.
The program aims to improve resident quality of life, safety, and care outcomes through facility enhancements and environmental upgrades. Eligible Licensed Nebraska Assisted Living Facilities (ALFs) can receive up to $500,000 through this grant. Applications are open until June 30, 2026.

Provide or support Memory Care, Dementia Care, or Alzheimer's Care services. Complete projects within the approved grant timeframe. Comply with all applicable Nebraska Department of Health and Human Services (DHHS) funding requirements.
Submit all required application and reimbursement documentation.
Wander management systems Nurse call system upgrades or modifications Visual cueing and wayfinding systems Sensory rooms and equipment Dementia-specific furniture Patient engagement technology Safe-access pathways and landscaping Non-Allowable Uses of Funds Real estate purchases or new building construction Major construction or large-scale remodeling Routine operating expenses or existing budget items Resident room/board or medical supplies Standard maintenance or general facility upkeep Marketing, advertising, or promotional campaigns Non-programmatic furniture or equipment Costs that are unreasonable, unnecessary, or above market rate without justification Only expenses incurred during the approved project period and within the approved project scope will be eligible for reimbursement.
Facilities are responsible for paying project costs upfront and then submitting eligible expenses to the Nebraska Health Care Association (NHCA) for reimbursement. The Rural Health Transformation Program is supported by the Centers for Medicare & Medicaid Services (CMS) of the U.S. Department of Health and Human Services (US HHS) as part of a financial assistance award totaling $218,529,075. 01 with 100 percent funded by CMS/US HHS.
