These payments may include sign-on bonuses, retention incentives, relocation assistance, and training or certification support tied to long-term workforce development. In exchange, clinicians receiving support must commit to a five-year service term, reinforcing long-term stability and sustainability in rural healthcare staffing.

Funding is structured as a cost-sharing model between the employer and the program, with the proportion of matching funds shifting over time to promote sustainability. In the initial two years, the program covers a majority share of financial incentives, while employer contributions increase in later years.

Reimbursements are capped based on clinician type, with maximum allowable amounts defined for various roles such as physicians, nurses, behavioral health professionals, and allied health staff. Payments are made after employers disburse funds to clinicians and submit documentation verifying compliance with program requirements.

Eligibility for participation is limited to healthcare providers that meet the Louisiana Rural Health Transformation Program’s rural definition. This includes organizations located in federally designated rural areas, facilities with CMS rural classifications, or providers that demonstrate that a majority of their patients reside in rural areas based on RUCA methodology.

Eligible organizations include rural health clinics, federally qualified health centers, critical access hospitals, small rural hospitals, and other healthcare providers such as behavioral health organizations, dental practices, pharmacies, and emergency medical services operating in rural settings.

The application process operates on an open enrollment basis, allowing providers to apply at any time; however, specific deadlines apply for each funding cycle. For the first budget year, applications must be submitted by late June 2026 to be considered for funding covering August 2026 through September 2027.

Applicants are required to submit organizational and program information along with a detailed Statement of Work and projected financial support schedule. Additional required documentation includes proof of rural eligibility, organizational credentials, and compliance with federal and state requirements.

Applications are evaluated on a rolling basis, with consideration given to eligibility, completeness, demonstrated workforce need, and alignment with program goals. The Louisiana Department of Health may request additional documentation during the review process and retains discretion to approve, partially fund, or deny applications based on available resources.

Successful applicants enter into a Cooperative Endeavor Agreement and must comply with ongoing reporting, auditing, and performance measurement requirements, including tracking workforce outcomes such as retention rates, recruitment growth, and time-to-hire metrics.

The program follows a defined timeline for the first funding cycle, including a question submission period in May 2026, an application deadline in June 2026, and an evaluation period extending into mid-July 2026. Awards are expected to be announced shortly thereafter, with funded activities beginning in August 2026.

While the program operates continuously, funding is allocated in budget years, and participants may have opportunities to apply for continued or additional funding in future cycles.
