Applicants from the U.S. territories or areas that do not have the functional equivalent of a SORH are still eligible to apply without completion of this The Rural Health Care Services Outreach Grant Program is a community-based program designed to expand, enhance, and sustain healthcare delivery in rural communities.
The overarching goals for the program are to: Expand the delivery of healthcare services to include new and enhanced services exclusively in rural Deliver healthcare services through a strong consortium, in which every consortium member organization is actively involved and engaged in the planning and Use community engagement and evidence-based or innovative, evidence- informed model(s) in the delivery Improve population health, and demonstrate health outcomes and sustainability Projects must fit into one of two funding tracks: Regular Track: The purpose of this track is to fund projects that expand access to healthcare in rural communities, improve the delivery of healthcare services, and strengthen resources and capacity in rural communities through collaborative consortiums.
These projects are community driven and focus on outreach and education activities targeting unique health needs in rural communities.
Goals include demonstrating improvement in health outcomes and creating long-term sustainability through funded Special Track: The purpose of this track fund projects that address the underlying factors that drive rural health disparities related to stroke, heart disease, chronic lower respiratory disease, unintentional injury, cancer, and maternal health.
The goals of Special Track projects are to demonstrate a collective impact to better manage conditions, address risk factors, and focus on prevention. See more information regarding project requirements in application instructions .
Eligible applicants must: Be a domestic public or private, nonprofit or for-profit entity with demonstrated experience serving, or the capacity to serve, rural underserved populations Represent a network composed of participants that: (1)Include at least 3 or more healthcare provider organizations; and (2)Include at least 66% of network members located in a HRSA-designated rural area Not previously received a grant under this program for the same or similar project, unless the entity is proposing to expand the scope of the project or the area that will be served through the project Nationwide and U.S. Territories Award ceiling: up to $250,000 per year Estimated number of awards: approximately 40 Award ceiling: up to $300,000 per year Estimated number of awards: approximately 10 Estimated total program funding: Links to the full announcement, application instructions , and the online application process are available through grants.
