Rural Texas Strong is sponsored by Texas Health and Human Services Commission (HHSC). The Rural Texas Strong program, funded through the federal Rural Health Transformation Program, aims to improve rural healthcare throughout Texas. HHSC will issue grants to rural hospital districts to enhance or create community-based prevention, wellness, and nutrition programs, or services aimed at improving chronic disease conditions. The program also focuses on supporting new physicians and practitioners through training, mentoring, and succession planning.

The Rural Health Transformation Program (RHTP) is a significant federal investment distributed to states to strengthen and modernize healthcare in rural communities.
