Division of Public Health Primary Care & Rural Health Programs Rural Health Transformation Program Rural Health Transformation Program The Rural Health Transformation Program (RHTP) serves to improve the health of rural Kansans by expanding primary and secondary prevention programs, securing local access to primary care, building a sustainable rural health workforce, enabling value-base care and harnessing data and technology.
This office works closely with the Kansas Rural Health Innovation Alliance in supporting rural health systems and the communities they serve to improve access and outcomes in health care.
Regional Partnership Grant Program Applications Now Available The $44 million RPGP grant enables eligible providers to participate in collaborative arrangements that promote preventive health care and address the root causes of disease, enhance rural providers’ efficiency and sustainability, attract and retain a high-skilled health care workforce, spark growth of value-based care models, and foster the use of innovative technologies.
Applications for the RPGP grant are due April 3, 2026. Rural Emergency Hospital Grant Applications Now Available The REH/CAP grant will provide facilities that have been converted or are in the process of converting to REH status with $15 million in available funds to make necessary facility renovations and provide other eligible providers with funds to make capital investments to transform how they serve their communities.
Applications for the REH/CAP grant are due March 20, 2026. Evidence-Based Practice Program Announces Webinar The Evidence-Based Practice Program, part of the State’s Rural Health Transformation Plan, will launch with a webinar on Monday, March 30, 2026, from noon to 1:00 p. m.
This webinar will introduce the Evidence-Based Program, which helps rural hospitals and clinics build and maintain the infrastructure to implement and sustain evidence-based practices and report on performance measures that demonstrate quality of care. The State has contracted with the University of Kansas Health System Care Collaborative to operate the Program.
The following details relating to the Program will be discussed during the webinar: Hospital and clinic eligibility criteria Payments to be made to participants Rural Hospitals: $100,000 infrastructure payment and up to $50,000 per quarter for July-September reporting Clinics (including RHCs, rural FQHCs, and CCBHCs): $50,000 infrastructure and $25,000 for July-September reporting Key provisions of the Program participation agreement The webinar will include a Q&A session.

KDHE Announces First Rural Health Transformation Program Grant Opportunities for Kansas Providers, Organizations The Kansas Department of Health and Environment (KDHE) posted Request for Applications (RFA) for its first two grant opportunities under the Rural Health Transformation Program (RHTP) in Kansas.
The application period for the Regional Partnership (RPGP) and the Rural Emergency Hospital (REH) Conversion/Transformative Capital Investment (CAP) grant programs opened Monday to eligible Kansas providers and organizations pursuing collaborative arrangements with one or more eligible Kansas providers.
Governor Laura Kelly today announced that Kansas has been awarded $221 million from the Centers for Medicare and Medicaid Services (CMS) to support the first year of the Rural Health Transformation Program. This funding will strengthen rural health systems, expand access to care, and improve health outcomes for Kansans across the state.
This funding award exceeds the base amount states allocated and reflects the quality of Kansas’ application. RHTP Application Submitted For nearly 900,000 Kansans, home is a rural community. Health care is the heart of these communities; those without local access to primary care services experience declining populations and shrinking economic opportunities for those who remain.
The fight to maintain services, therefore, is literally a battle to preserve these communities. With significant stakeholder engagement, the State has developed an ambitious Rural Health Transformation Plan and applied with the Centers for Medicare & Medicaid Services (CMS) for funding to help realize this plan. KDHE Seeking Public Comment The Kansas Department of Health and Environment (KDHE) is seeking public comment to inform the state’s strategic approach to applying for and implementing the RHTP. KDHE is committed to pursuing an approach that is both expeditious and effective, directing limited resources to high-need areas with a focus on sustaining critical, high-quality health care access and services for rural Kansas. H.
R. 1 Section 71401 of the One Big Beautiful Bill Act (OBBBA), enacted into law on July 4, 2025, establishes the Rural Health Transformation Program (RHTP) and created a national $50 billion fund, to support this initiative. All 50 states are eligible to apply for this funding.
The Centers for Medicare & Medicaid Services (CMS) will establish an application process and due date. Additionally, CMS must approve or deny applications no later than December 31, 2025. Applications are required to be submitted in early November 2025.
