Requires a completed application, a typed letter detailing how funds will assist, postgraduate practice plans, current connections to Northwest Oklahoma, and three letters of support. Current medical students need letters from one Osteopathic Medical College Faculty, one Osteopathic Physician, and one Community Leader. Students accepted to a COllege of Osteopathic Medicine (COM) and beginning classes in August need letters from one Undergraduate Faculty Member, one Osteopathic Physician, and one Community Leader. A current transcript indicating cumulative GPA and class ranking is also required.
