The Grant Application and Programmatic Risk Assessment are to be completed for every program/grant.

At the time of application, all Bureau grantees must submit for each grant program the following documents: Uniform Application for State Grant Assistance (pdf) FY22 Second Chance Act: Youth Offender Reentry DJJ Continuation Application CSA Budget/Tracking System and Registration Information (docx) GATA Training Link: IDHS GATA Technical Assistance Learning Management System The completed documents are to be emailed by May 5, 2021 by 12:00pm (noon), to the Bureau email address: DHS.
