This program targets the nation's neediest populations and geographic regions. Areas with an April 1, 2026 project period start date are eligible to apply. Funding is available for Community Health Centers (CHC), Migrant Health Centers (MHC), Health Care for the Homeless (HCH), and Public Housing Primary Care (PHPC).
Applicants must serve an entire service area listed in Service Area Announcement Table . Applicants must be a public or nonprofit private entity, such as a tribal, faith-based, or community-based organization that proposes to offer comprehensive primary care services to an entire service area. Additional eligibility requirements are listed in the application instructions .
Applicants may apply for the amount of the total Service Area Announcement Table for the proposed service area in total cost per year. Project period: Up to 4 years Links to the full announcement and online application process are available through grants. gov .

The grants. gov application must be submitted by September 29, 2025, and the HRSA EHB supplemental information must be submitted by October 29, 2025. Application resources, forms, and frequently asked questions are available on the Technical Assistance website .
