* Indicates required field Question about application process *## We fund services for Senior Citizens, with a priority for nonprofit organizations supporting residents of Cupertino, Los Altos, and Mountain View with the following focus areas: Education, Research, & Outreach** Grants focus on areas relevant to our mission, including adult day services, memory care, stroke recovery, grief support programs, services for the visually impaired, caregiver support, and senior education including digital literacy.
In addition, we fund services that encourage healthy lifestyles, and client transportation to and from all the services mentioned above. We also fund education, research, and outreach that build awareness of issues important to seniors in our community. These grants are supported by generous donations from residents at The Forum at Rancho San Antonio and surrounding communities.
## Medical Care & Healthy Lifestyles * Bay Area Older Adults(BAO) provides free socialization programs for older adults and people with disabilities, dementia and their caregivers, many of whom are socially isolated due to retirement, divorce, death of loved ones and friends, relocation and disability.
* Pacific Hearing Connectionis a Los Altos-based nonprofit supporting those in our community who are not able to afford seeing a hearing specialist or buy hearing aids. Approximately 75% of their clients are over 65. Services include initial screening, wax removal, fitting hearing aids, counseling, and follow-up care.
Our grant enables them to stock supplies and provide staff for screening and counseling at community healthcare events. * VISTA Center for the Blind and Visually Impairedprovides vision loss rehabilitation through their Safe and Healthy Living Program, which has proven effective in helping visually impaired vulnerable seniors maintain their independence, live life with dignity and avoid early institutionalization.
VISTA strives to empower individuals who are blind or visually impaired to embrace life to the fullest through evaluation, counseling, education, and training.
**Programs at the Forum Health Care Center** * **Stroke recovery program** at the using LiteGait and the Interactive Metronome technologies including state-of-the-art equipment providing weight-bearing assistance for fall-free therapeutic activities, recovery support and staff training.
* **Equipment and staff training for use of iN2L** (It’s Never Too Late) new technology to improve quality of life for those in memory care at the Forum. Emerging technologies have shown potential to find a kinder and gentler path through the turbulent journey of such disease. These programs introduced new technologies into the recently-opened Daffodil Ridge Memory Care Center at the Forum.
The Forum Memory Care Center team has been nationally recognized for its work in incorporating the iN2L technology into programs for memory care residents. This included recognition given to only six programs out of 3,700 for the team’s innovative use of tablets, large screens and a content library to keep memory care residents engaged with their families during the pandemic. * **Fitness equipment for Assisted Living**.
This equipment will support strength, balance, energy and endurance for those in Assisted Living, helping to maintain independence, and carry out everyday activities.
Please see additional resources at: The Forum at Rancho San Antonio Wellness ## Social Services Grants (Adult Day Services, Caregiver Support, Digital Inclusion, Bereavement Support, Senior Case Management) * Avenidas Rose Kleiner Center in is a state-of-the-art adult day health care center in Mountain View. They provide a safe, therapeutic, and supportive environment for adults with cognitive and physical challenges.
Caregiver support resources and services are an integral part of their program. * Community Services Agencyin Mountain View provides support services for local senior citizens in need. CSA is a social safety net agency providing vital social services to residents of Los Altos, Los Altos Hills and Mountain View.
CSA provides critical support services that preserve and promote stability, self-reliance and dignity. * Compassion Week Los Altos brings residents together for community building by creating hands-on volunteer opportunities for people of all ages and abilities. Our grants target senior citizen beneficiaries by engaging older adults as volunteers and creating a service project benefiting a senior citizen-serving recipient agency.
* AnewVista CommunityServicesfocuses on providing services for digital inclusion for at-risk seniors. ANVCS provides technology training programs for seniors in underserved areas, including in-person and remote technical training and support specialized for senior’s unique needs, in a safe and supportive community environment.
ANVCS programs include regular access to technology experts, building confidence and enhancing seniors’ familiarity with essential everyday technology and building confidence. * Live Oak Adult Day Servicesseeks to provide socialization and cognitive stimulation to seniors in Santa Clara County and to support family caregivers in their efforts to maintain senior dependent loved ones in the family home.
* KARAprovides grief support programs and services to families and individuals, including seniors. The grant supports bereavement services to seniors. Services are offered in group settings and individually.
KARA’s vision is to see people of all ages compassionately supported on their journey through grief so they can move toward renewed hope and meaning. KARA has received a second follow-on grant for continuing support of this program. * Additional grants were made in new collaboration with theLos Altos Mountain View Community Foundationas part of the Forum Charitable Fund’s expanding outreach into our local communities.
## Outreach, Education & Research Grants * SASCC(Successful Aging Solutions & Community Consultation) is expanding its hyper-local community news and information to the City of Cupertino with the Outlook newspaper. * We have a collaboration with Stanford researcher V. J.
Periyakoil, MD, Director of **AGE Research Center**, studying the impact of isolation on seniors during the COVID-19 pandemic lockdown. http://aging. stanford.
edu * Cupertino Library Foundationsupports the development and delivery of a community speakers' series about senior issues. * Fremont Union High School District Adult School's memoir writing class for seniors. The Forum Charitable Fund is actively accepting proposals for sponsorships in the areas outlined above.

Emergency Food and Shelter National Board Program is sponsored by Federal Emergency Management Agency (FEMA). The Emergency Food and Shelter Program (EFSP) provides federal funding to supplement the work of local social service organizations to provide emergency food and shelter to people experiencing, or at risk of experiencing, homelessness or hunger. The National Board, chaired by FEMA, allocates funds to local boards (like the one in Clark County, administered by United Way of Southern Nevada) which then distribute funds to eligible local recipient organizations (LROs).
