The goal of this NOFO is to provide support for the Republican AIDS Center (RAC) of Tajikistan to maintain high-quality HIV services to achieve HIV epidemic control. The strategies and activities under this NOFO include 1) Development, implementation, and coordination of effective case finding, index testing services, and linkage strategies for people living with HIV (PLHIV) in Tajikistan, targeting key populations (KPs); 2) Adoption and national expansion of evidence-based HIV service delivery models that optimize treatment retention, adherence, and viral suppression; 3) Implementation of measures to reduce stigma and discrimination in HIV prevention and treatment services to ensure that services are client-centered and appropriate for KP; and 4) Implementation of program monitoring, surveillance, and continuous quality improvement to track trends in the HIV epidemic response across locations and populations. The long-term outcomes expected to be achieved are as follows: a) Improved diagnosed early and immediate initiation on antiretroviral therapy (ART); b) Sustained retention and viral suppression among PLHIV; c) KPs are effectively accessing client-centered HIV services; and d) Sustained ability to effectively respond to the epidemic and achieve epidemic control. The NOFO will begin on September 30, 2020 and builds on NOFO CDC-RFA-GH15-1514 which ends September 29, 2020.
