## Temporary Respite Caregiver Support Caring for a loved one can often require a helping hand. Short-term financial assistance is now available to help relieve the caregiver burden. Together with the San Francisco Department of Disability and Aging, IOA is offering temporary grants for in-home care and Adult Day Program services.

Certified clinicians supervise caregivers who work one-on-one with clients. The care managers meet clients and their families to design and implement a personalized home care plan.
Licensed, bonded, and insured caregivers are on call 24 hours a day, 7 days a week.
Clients and their families can customize their home care, choosing services including: - Comprehensive assessments - Hourly or 24/7 live-in care and overnight assistance - Communication with family, friends, and support - Home safety evaluations - Meal planning and preparation - Escorted transportation, errands, and accompaniment & light housekeeping - Medication management - Assistance with long-term care planning### All-Inclusive Health Services Institute on Aging sponsors ON LOK that founded the program PACE (Program of All-inclusive Care for the Elderly) created to empower seniors who are ill or disabled to live in their own home, whether home is a family residence, apartment, retirement village or hotel room.
Participants will have access to medical care, home care, transportation, meals, and more. Services are provided at the center or in the safety of your home within a caring community committed to your health and well-being.
Covered Services Include, but are not limited to: All medical care Dental, Vision, Hearing Transportation Day Center Activities Home Care Meals The healthcare plan covers all Medi-Cal and Medicare services, plus more. ### Community Living Fund For people struggling to get by on their own, Institute on Aging can become a support system of caring people who will go out of their way to help.
IOA’s community living services assist people with a limited income transition out nursing homes and care facilities and get back to living independently. ### Enrichment Center at the Presidio Institute on Aging's Enrichment Center at the Presidio provides a much needed physical and mental stimulation for isolated aging adults to remain active and engaged within a caring community.
The staff specializes in supporting those with physical and cognitive challenges, while providing their supportive family members with respite opportunities.

### On Lok (PACE) On Lok founded the Program of All-Inclusive Care for the Elderly (PACE), a nationally recognized program created to empower seniors to age with dignity in the community, instead of a nursing home. On Lok has successfully contracted with Institute On Aging (IOA) to offer On Lok PACE at IOA. Our full-service healthcare program includes medical, home care, transportation, meals, and more.
Services are provided at our center or in the safety of your home, within a caring community committed to your health and well-being.
Covered services include, but are not limited to: - All medical care - Dental, vision & hearing - Transportation - Day center activities - Home care - Meals Our medical program includes: - Doctor visits - Medical specialists - X-rays - Emergency / Urgent care - Hospital care - Medication - Nursing home care### Community Living Fund IOA’s Community Living Fund is a short-term, intensive care management program designed to helps lower-income San Francisco County residents transition out of hospitals and care facilities so they can live independently.
The CLF helps aging adults and people living with disabilities transition out of care facilities if they do not belong there, or do not wish to be there. CLF assists individuals by connecting them to medical and psychosocial services that will support independent living.
We serve as the guiding hand within an extended network of programs and services, helping individuals find secure living arrangements, connecting them to medical and psychosocial services to support independent living and helping them manage their care. IOA’s Community Living Fund also advocates for individuals who are willing and able to continue living in the community, but who are at risk of institutionalization.
Our staff works to ensure that individuals are connected to all available community resources, stable housing, and appropriate healthcare services to support their ability to continue living at home safely, including proper medical equipment. Potential clients must first undergo an eligibility screening process through the City of San Francisco’s Department of Disability and Aging Services.
Afterwards, our care managers will assess client’s needs, develop a service plan, and then work to maintain or return them to a safe, stable, independent way of life. ### Adults With Disabilities Home-Delivered Meal Program The Adults with Disabilities Home Delivered Meal Program is available for adults with disabilities who have no meal support and are unable to provide meals for themselves.
The program provides nutritious meals to low-income disabled adults between the ages of 18 and 59 who would be at risk without a home delivered meal and/or have difficulty accessing nutritionally balanced meals on a daily basis.
### Elder Abuse Prevention The Institute on Aging’s Elder Abuse Prevention Program provides education, outreach, advocacy, and strategic partnerships to prevent and respond to the abuse of older adults and adults with disabilities. We provide presentations to professionals on how to identify, prevent, and report elder abuse. We provide presentations to older adults on how to protect themselves against abuse and scams.
Our programs and services educate and prevent elder abuse by: Training service providers and older adults on how to detect, prevent, and report elder abuse Creating public awareness campaigns Providing expert speakers specializing in: – Elder abuse – Dependent adult abuse – Scams & financial abuse Coordinating the San Francisco Elder Abuse Forensic Center and Multidisciplinary teams Advocating for new policies and services Participating in the San Francisco Family Violence Council### Home Safe Program Home Safe is a program intended to support the safety and housing stability of individuals within Adult Protective Services (APS), which supports victims of abuse and/or neglect who are either adults age 60+ or adults ages 18-59 who are living with disabilities.
Home Safe provides intensive case management and other supportive services to individuals experiencing or at imminent risk of homelessness due to elder or dependent adult abuse, neglect, self-neglect or financial exploitation. ### EDUCATION & Training for Caregivers Institute on Aging offers education on various topics that concern our aging community to help individuals be a more effective and helpful caregiver or loved one.
Educational curriculum is developed by medical experts, community leaders, industry experts, and psychotherapeutic professionals for a caring and compassionate presentation. Institute on Aging fosters a community that is knowledgeable about the current trends in aiding aging adults and people with disabilities.
They host a number of conferences, talks, workshops, and educational sessions that bring new discoveries and techniques to the forefront. All educational and training programs are online.
### Institute on Aging (IOA) Case Management Institute On Aging (IOA) Community Case Management helps older adults, adults living with disabilities, or their caregivers coordinate all the services they may need in order to continue to live in the place they call home.
IOA Community Case Management is a partnership between the City of San Francisco’s Department of Disability and Aging Services (DAS) and several case management providers throughout San Francisco County. IOA is one of many non-profit providers of this service.
