The ACT on Health Equity Community Solutions Challenge Grant is sponsored by AstraZeneca Foundation. This grant aims to enhance health equity by funding community-based nonprofit programs that address nutrition, healthcare access, and lung health for historically marginalized populations.

AstraZeneca US has made a long-term commitment to identify and invest in community-based solutions addressing health disparities among communities that have experienced barriers to care and opportunity. Through our ACT on Health Equity Community Investment programs, we’re continuing to transform the way we do business to improve access, affordability, and outcomes for patients in the disease areas and communities we serve.

com/content/az-us/sustainability/community-investments. html)Community Solutions Challenge The Community Solutions Challenge (CSC), launched in 2021, works to advance health equity through the support of community-based 501(c) nonprofit organizations with programs prioritizing the social, cultural and linguistic needs of communities that have experienced barriers to care and opportunity. To date, CSC has provided more than $3.

7 million to over 160 local nonprofit programs working to address health inequities and advance opportunities in STEM among communities that have experienced barriers to care and opportunity. In 2025, CSC will provide up to $1. 5 million in funding to new and existing programs.

**The 2025 application window is now closed. ** ## Community Solutions Challenge Awardee Spotlight Mobile Care Chicago’s mission is to deliver no-cost medical and preventative care, education, and support to low-income children and families within their communities aboard mobile medical clinics.

For 24 years, Mobile Care Chicago has used partnerships with local schools to provide a convenient and trusted location for local children who have complex needs but whose parents may not have the time, transportation or work flexibility to access more distant brick-and-mortar clinics.

### Parkland Health Foundation Early detection of breast cancer saves lives, but far too many underserved and uninsured women in Dallas County go without essential screening and diagnostic services due to barriers like cost or lack of transportation. Parkland Health believes that the key to reversing this trend is ensuring that routine screenings are accessible and affordable, reaching high-risk women where they live, work, and learn.

### Union Station Homeless Services For over 50 years, addressing food insecurity among people experiencing homelessness has been at the center of Union Station Homeless Services' programming.

Now more than ever, they are stepping up to meet the growing need for nutritious food within their community and those they serve, and this year they are on track to serve nearly 600,000 meals to people across Northeast Los Angeles and the entire San Gabriel Valley.

The meals not only fill hungry bellies, but also serve as an opportunity for them to teach about nutrition, engage people in their services, and attract community volunteers to assist in reducing the stigma surrounding homelessness. ### [](https://www. astrazeneca.

html)National Strategic Collaborations Nationally, ACT on Health Equity further supports our strategic collaborations with nonprofit organizations that have a focus in the disease areas and communities we serve to: * Increase understanding and awareness of the social, cultural and linguistic needs of communities that have experienced barriers to care and opportunity; * Improve reach & cultural responsiveness of patient programs; and * Address social determinants of health Through National Strategic Collaborations (NSC), 40 national organizations have received a total of $11.
