National Cardiovascular Health Program (2023–2028) | odphp. health. gov The National Cardiovascular Health Program aims to implement and evaluate evidence-based strategies contributing to the prevention and management of cardiovascular disease in populations at a higher risk.
The National Cardiovascular Health Program is a 5-year cooperative agreement. The current program runs from 2023 to 2028.
Awardees of the Innovative Cardiovascular Health Program implement team-based care, in which doctors and nurses work with pharmacists, community health workers, and others outside of medical settings, to manage a patient's risk factors to prevent, detect, control, and manage hypertension and high cholesterol as well as link community resources and clinical services that support comprehensive bidirectional referral systems that promote optimal health outcomes.
