The foundation prioritizes prevention and promotes strategies that reduce health disparities within our community. Luke Hospital in east Pasadena. After the hospital was sold to a for‑profit company in 1985, the foundation could no longer fund it. Honoring a commitment to preserve $2.

8 million raised for community health, the organization was reorganized in 1986 as The Patron Saints Foundation with a mission to provide health‑focused grants to nonprofit organizations serving the West San Gabriel Valley. Today, The Patron Saints Foundation is committed to ensuring that all individuals in the West San Gabriel Valley area are healthy and able to reach their full potential.

Through its annual grant program, the foundation supports initiatives that focus on long-term community healthcare services, including primary care, and de n tal and mental health services. Funding is directed toward programs that are collaborative, resource-leveraging, and culturally responsive.

Patron Saints Foundation seeks proposals that: Emphasize community healthcare services including primary care, dental and mental health services Demonstrate clear and measurable impact and outcomes on individuals in the West San Gabriel Valley area Address well-documented community health needs Are culturally relevant and responsive to racial and ethnic disparities Remove barriers to health services for isolated and vulnerable populations Collaborate with other organizations to enhance impact and efficiency Grant Decisions: Early June 2026 Grant Amounts: Up to $25,000 To be eligible for funding, organizations must: Be located in or provide significant services within the West San Gabriel Valley as defined as follows: the cities of Alhambra, Arcadia, Duarte, El Monte, La Canada Flintridge, Monrovia, Monterey Park, Pasadena, Rosemead, San Gabriel, San Marino, Sierra Madre, South El Monte, South Pasadena, Temple City and the unincorporated areas of Los Angeles County known as Altadena and South San and (2)(b)Gabriel.

Be a tax-exempt organization under Section 501(c)(3) or 170(c)(1) or (2)(b) (Private Foundations under 509(a) are not eligible). Generally, have at least two years of operational history with a record of effective service (fiscally sponsored programs may have different requirements). Have completed all previous Patron Saints Foundation grants; organizations with an open or non-compliant grant are ineligible.

Adhere to the Non-Discrimination Policy outlined below Non-Discrimination Policy PCF does not make grants from discretionary funds to organizations that discriminate based on: Actual or perceived rage, age, color Gender identity, marital or parental status Military service, national origin Physical or mental ability Political affiliation, sexual orientation, or religion Individuals or households Organizations with open or non-compliant PCF grants Activities intended to influence legislation or elections Individual public schools or their support groups (e.g., PTA/PTO, booster clubs); PCF supports PUSD schools only through the Pasadena Educational Foundation (PEF) Sectarian religious organizations or churches for religious purposes (community service programs may be considered if open to all and meet other criteria) Agencies not directly performing services aligned with PCF’s mission (no regranting) Building improvements on leased property (exceptions may be made for public entities or long-term leases at the board’s discretion) Grant Reporting Deadlines & Compliance Reporting requirements must be met to remain eligible for future PCF grants.

Agencies must also comply with PCF’s Grand Recognition Guidelines (see below). Specific reporting deadlines and formats will be provided upon grant award. PCF actively promotes its grants and supports grantees in doing the same. Grantee Responsibilities: Required Language: “This project was funded [or funded in part] by a grant from the Pasadena Community Foundation.

” Naming Convention: Use “Pasadena Community Foundation” on first reference; “PCF” may be used thereafter Logo Usage: Use the provided “Grantee Organization” logo or standard PCF logo with written acknowledgement Shared Coverage: Send copies of any media or press coverage mentioning the PCF grant to the foundation – media@pasadenacf.
