Traumatic Brain Injury Fund Traumatic Brain Injury Fund To begin the application process please click on ‘Apply Now’. You will be asked a series of questions and required to upload copies of eligibility documents. After you click ‘Submit’ your application will be sent to your healthcare provider for the medical documentation.
Once the TBI Fund receives the completed application, your eligibility will be reviewed and a determination letter will be sent to you. Before you start the application process, please make sure you have the following documents ready to be uploaded: Three (3) most recent months of bank statements. A copy of driver’s license, state ID, government issued correspondence or current utility bill.
