Please be prepared to answer the following questions: Geographic area being served (not an attachment) Mission Statement of Organization Please note that the online application requires the following attachments: Copy of IRS Determination Letter Financial Statements (most recent year-end) Program Budget (detailed) List of any previous support within the last five years from Truist, Truist Foundation or Truist West Virginia Foundation, including the date, dollar amount, and purpose of previous contributions.
(if applicable) Schedule of any anticipated funding requests from Truist for this year. (if applicable) Any other needed supporting documentation such as letter of support, community strategic/master plan, etc. (optional) Attachments can be in Adobe PDF (preferable), Microsoft Word or Excel format. Please do NOT include compressed ZIP files or images (JPEG, PNG, TIF, etc.).
