WindSwell Inclusive Communities Grant Cycle is sponsored by Community Foundation of Grant County and WindSwell Foundation. This grant opportunity focuses on creating more inclusive communities for individuals with disabilities.

WindSwell Foundation Accessible Communities Fund | Kosciusko County Community Foundation In an effort to provide support for building inclusive communities for those with disabilities, the WindSwell Foundation and Kosciusko County Community Foundation have partnered to offer the WindSwell Foundation Accessible Communities Fund initiative.

Applications will be accepted on an ongoing basis and will be awarded through this competitive grant application process. Awards of up to $5,000 will be made until all funds have been allocated. Charitable organizations serving Kosciusko County residents are eligible to apply for projects and programs that meet the Guiding Principles of the fund.

The first time you apply, the Community Foundation staff will need to create a user login for you. Please reach out to us for assistance. Grant seekers are required to contact Alex Hall ( alex@kcfoundation.

org ), Vice President of Programs, to set up a time to discuss your proposal prior to submitting a formal application online. Fillable PDF Word Document Organizations that have not applied for other funding from the Community Foundation within the past 12 months will be required to upload the additional documentation listed below. IRS Determination Letter (required for 501c3 organizations) List of board members with city/town of residence List of staff members with city/town of residence What financial documents should I include with my application?

Nonprofit organizations with annual operating budgets of $100,000 or more are required to submit a Statement of Financial Position (Balance Sheet) and Statement of Activities (Income Statement) for the organization’s most recently completed fiscal quarter. Examples of both documents are below. Your organization’s most recent 990 is also requested, but not required.

Government agencies, including public schools, are not required to submit financial documents. Sample Statement of Activities Sample Statement of Financial Position Weeks 1-2 after application submitted Staff reviews applications that have been received for eligibility and completion. Staff may follow up with applicant with any clarifying questions that the reviewers may have.

Reviewers receive applications and meets to discuss recommendations to be sent to the Community Foundation's board of directors for approval. Weeks 4-6 after submission Board reviews the recommendations and final decisions are made. Letters are mailed within one week of the board’s decision to notify applicants.

Grant agreements are included with the letter for approved requests. Grant agreements due for successful applicants. Grant expires and successful applicants submit a final narrative report describing the impact of the project with documentation verifying expenditures.
