The Community Impact Grants program directs hospital funds to local organizations serving vulnerable populations. This funding aims to promote optimal health by addressing social needs, connecting social and clinical care, and reducing health inequities. Non-profit organizations operating in—and serving the residents of—Kent County, Michigan are eligible to apply.
Grant-funded programming must align with Trinity Health's Mission, Vision and Values , and must address at least one of the top 5 priorities of the most recent Kent County Community Health Needs Assessment (CHNA): Review Process and Awards Applications are reviewed and approved by the Trinity Health Grand Rapids Community Health & Well-Being (CHWB) Staff, the Community Impact Grants Review Committee, and the CHWB Steering Council.
Community Impact Grants are awarded for up to $40,000 each, and no program may receive funding more than three consecutive years. Grants of $10,000 or less are awarded if funds are available after principal grants are approved. The application process for the 2027 calendar year will launch in October 2026.
