Community Support Grants | Trinity Health Michigan Trinity Health Michigan Medical Groups Probility Physical Therapy Physical therapy services Athletic development services Community Impact Grants allow Trinity Health Michigan to invest in community-based efforts designed to respond to priority needs identified in our tri-annual community health needs assessment and implementation plans of the various communities we serve.
We achieve this by partnering with community-based partners, organizations, and coalitions with the intention to serve vulnerable members of our communities and work to improve the overall health and well-being of Trinity Health Michigan communities. 501(c)3 organizations are encouraged to review and apply. It is important to note that funding pools are managed and overseen by the local hospital and their governing bodies.
