Health (Grants for improving the effectiveness, responsiveness, and equity of health care)

Health Grants is a program from The New York Community Trust that funds projects improving the effectiveness, responsiveness, and equity of health care for marginalized and underserved communities on Long Island. The program supports efforts to address health disparities, expand access to comprehensive care for uninsured or underinsured individuals, provide early intervention and disease screening, and advance maternal and child health.

Health grants are accepted and reviewed during cycle 3 with an August deadline. In Nassau County, eligible applicants are hospitals and Federally Qualified Health Centers; in Suffolk County, eligible applicants include hospitals, health care facilities, and nonprofits aligned with program goals.

The Trust funded Harmony Health Care Long Island to expand the availability of a mobile clinic to bring healthcare directly to underserved communities.

To improve the effectiveness, responsiveness, and equity of health care for marginalized and underserved communities and populations. Address existing health disparities and improve access to quality healthcare. Provide access to comprehensive and coordinated care for those who remain uninsured or underinsured.

Provide screening and early intervention for effective treatment of disease. Support maternal and child health care—including services, prevention, and education. Health grants are accepted and reviewed during cycle 3 (August deadline).

Eligible applicants in Nassau County include hospitals and Federally Qualified Health Centers. Eligible applicants in Suffolk County include hospitals, health care facilities, and nonprofit organizations with projects that align with our program goals.

Grants are made for one year at a time; proposals submitted for renewed funding are reviewed on a case-by-case basis, but only after a final report on the use of the previous grant has been received and approved by Trust staff. For questions about this program area, contact Melissa Greenberger .
