For completed applications, please allow 3-4 weeks to receive your reimbursement. All applicants must submit a W-9 form with their application regardless of if they have received reimbursements in the past. Please follow the instructions on the application to apply to the program.
USDA National Organic Program Accredited Certifying Agents Agents currently USDA-accredited and authorized to certify operations to the USDA organic standards. W-9 form : Request for Taxpayer Identification Number and Certification Requirement for reimbursement from the Oregon Department of Agriculture (ODA). Complete this form if you have made any changes or have never received payment from the ODA.
